25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 04/11//2022. LPA arrived at the facility at 11:30 AM and met with Manoushak Mkroyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Gevorg Mkroyan and Shnorik Zargaryan, Licensee’s Assistants. Per licensee, facility hours are Monday to Friday from 6:00 a.m. to 6:00 p.m.
This is a two story home which consists of 5 bedrooms, 4 full and 2 half bathrooms, living room, dining room, 3 kitchens, attached converted garage to guest room, front yard and backyard (fenced). There is a fireplace in the living room which has been blocked off to prevent access to children. Per licensee the children use the half bathroom in the hallway, living room and 2 bedrooms in the first floor, and front yard (fenced). Per licensee areas off limits to children and parents include: 3 Kitchens, dining room, 1 bedroom and 1 full bathroom in first floor, entire second floor which consist of 2 bedrooms and 2 bathrooms, attached converted garage to guest room, 1 bathroom inside the guest room, and backyard fenced. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 11:45 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room (main daycare room), LPA observed children's table, chairs, and toys in the living room. LPA observed all electrical outlets were covered in living room. LPA observed a safety gate was installed to the kitchen to make the kitchen inaccessible to the children. LPA also observed a safety gate was installed in the hallway of bedrooms. Next LPA toured the daycare bedroom adjacent to the living room and observed 2 pack 'n plays and toys in the bedroom, Next LPA toured the middle daycare bedroom and observed napping cots and toys in the bedroom. Next LPA toured the half daycare bathroom in the hallway and did not observe any hazards. LPA observed doors that leads to the dining room and gate which leads to the kitchen were closed to prevent access of children to the off-limit areas.REPORT CONTINUES ON NEXT PAGE 1 of 4 |
