On 11/2/22 Licensing Program Analysts (LPAs) Austin Estrada and Jeanette Estrada conducted an unannounced Required Annual Inspection at the above facility. LPAs met with Facility Representative Ariana Ramirez , who provided a tour of the facility. Director Blanca Aponte arrived at a later time and continued the tour of the facility. Per Director, the hours of operation are Monday - Friday 6:30AM to 6:30PM. The facility is located on the same premises of a preschool program (facility #198019910). There is one classroom (classroom 4) where care is provided to school age children.

Upon arrival LPAs observed no children in the school age program. At 1:00PM LPAs observed eight children and one staff present in the school age classroom. Currently there are 10 children enrolled. All individuals present have obtained a criminal record clearance or criminal record exemption. The following was observed during the tour of the facility:

All areas identified on the Facility Sketch were inspected.

LPAs observed the classroom to be clean and safe. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. LPAs also observed the floors and carpeting to be safe and clean. Per Director, there are no children that require medication. There is a medication box in the classroom where it could be locked and stored appropriately if needed.



At the moment, one bathroom is shared for both the male and female children as one of the bathrooms is in repair. Per Director, a teacher is with the children at all times while they are using the bathroom and the males and females use the bathroom at separate times. The toilet and hand washing facility are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free.

Per director there are no poisons located at the facility. The Directors office is used as an isolation area for ill children. Ill children are also provided a separate bathroom in which they may use. Parents are contacted immediately when children are determined to be ill. Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides breakfast and afternoon snacks to children in care. If a child is staying full time they will be provided with breakfast, lunch, and an afternoon snack. The kitchen, food preparation areas, and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. LPAs also inspected the refrigerator and observed it to be clean and in operable condition. LPAs observed food in the refrigerator not to be expired.

There is drinking water available in the classroom and children are provided with plastic cups. Trash receptacles have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, and other items that are dangerous to children are located in a locked cabinet inaccessible to children in care. Facility has a functioning carbon monoxide/ smoke detector that meet statutory requirements. LPAs observed a fire extinguisher serviced in September 2022. LPAs reviewed the Earthquake & fire drill log last conducted in September 2022. Per director, there are no weapons located at the facility.

Outdoor play equipment was observed to be in good condition, free of sharp, loose, or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. Areas around and/or under climbing equipment, cushioning material to absorb a fall. LPAs did not observe any bodies of water at the facility during this inspection.

LPAs reviewed sign in and out forms to ensure children will be signed in and out. The facility uses an electronic application called “Procare” in which parents/ authorized representatives are able to sign in and out using a tablet.

Facility provides transportation to and from school to children in care. There is currently one vehicle that is being used to transport children. Per Director there is currently one child that gets transported to school in the morning, but there are more children that are transported after school back to the facility. Per Director, the parents advise the teachers through the “Procare” application if their child does not need transportation from the school back to facility. LPAs observed the transportation vehicle to be in good condition. LPAs observed restraint systems to be in working condition. LPAs did not observe any children in the vehicle at the time of inspection.

Children’s records were reviewed for Emergency Card with signed medical consent, Immunization Records, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, and signed Admissions Agreement. All documents were observed and complete. Staff records were reviewed at for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. All documents were observed and complete.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence at the facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

No deficiencies were cited by Title 22 of the California Code of Regulations and Health & Safety Codes. Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted, and a copy of this report was provided to the Director.
