Licensing Program Analyst (LPA) Judy Mora and Licensing Program Manager (LPM) Claudia Guangorena conducted an unannounced Case Management inspection on this day. Licensing staff met with, Marine Adamyen, Licensee's assistant, who guided licensing staff on a tour of the facility. There were 14 children present at the time of visit. CTS Language Link was used as the Licensee’s Assistant and children speak Armenian, Name and ID number, Ghahramanyan #15348 and Collette #8919. Licensee, Taron Mayilyan was not present at the time of arrival and arrived at approximately 2:20 PM.

At approximately 1:52 PM, Licensing staff observed 05 children present in the front room and 09 children present in the nap room.

The purpose of this visit was to ensure that the Licensee is in compliance with Title 22 regulations and the Health and Safety Code. Children’s files were reviewed to ensure that a LIC 9224 Acknowledgment of Receipt of Licensing Reports is on file for the Non-Compliance conference held on 11/09/2022.

Licensing staff observed that there were no LIC 9224's present in any child's file. Licensee indicated that the signatures were obtained but he does not know where the forms are.

Licensing staff also provided Licensee with a copy of the family child care website advertisement. Licensee indicated that he did create the advertisement for his and his mother's child care. Licensee indicated that he used Wix.com for the creation of the advertisement.

Licensee was informed that all advertisements must have their assigned license numbers. Licensee was also informed that the advertisement must indicate it is a family child care home and not a childcare center.

The deficiencies listed on the following pages were observed by the Licensing staff and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809d.

Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Licensee, Taron Mayilyan. Appeal rights explained & provided.
