25 | REQUIRED - 1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 05/12/23. LPA arrived at the facility at 12:40 PM and met with Karine Petrosyan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, was Harutyun Khachatryan, Licensee’s Husband/Assistant. Per licensee, Karine Petrosyan facility hours are Monday to Saturday from 7:00 a.m. to 11:55 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home consists of 3 bedrooms, 3 restrooms, living room (day care room), dining room, kitchen, guest house in the backyard, laundry room, patio, front yard, and backyard (fenced). There is a fireplace in the dining room which has been blocked off to prevent access to children. Per licensee, parents enter the home through the main entrance door which leads to the living room (main day care area).
The children use the bathroom in the hallway, living room (day care room), dining room, and backyard (fenced). Per licensee, areas off limits to children and parents include: 3 bedrooms, 2 bathrooms, kitchen, guest house in the backyard, patio, laundry room and front yard. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
At 1:00 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room (main daycare area) where observed four children were napping. LPA observed a child proof safety gate was installed to the living room to make the rest of the home inaccessible to the children. LPA observed children toys, tables and chairs the living room. LPA observed all electrical outlets were covered in the living room. Next LPA toured the kitchen and observed a child proof gate was installed to the kitchen to make the kitchen inaccessible to the children. Next LPA toured the dining room and observed a dining table, chairs, sofas and a coffee table in the dining room. LPA observed fireplace was barricaded to make it inaccessible to the children.
