On 7/28/21 Licensing Program Analyst (LPA) Jeanette Estrada conducted an unannounced annual inspection to the above facility. LPA arrived at the facility at 1:44 PM and met with Karen Munoz, Licensee who guided analyst on a tour of the facility. Per Licensee, there are 6 children that are currently enrolled. A current children’s roster was available for review. There were 4 children, the Licensee and her assistant (Staff #1) present upon arrival. The Licensee holds a foster license through DCFS. Operating hours are: Monday- Friday 7AM-6PM
Individuals who reside in the home were noted and discussed. All adults present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in the licensed childcare home. Licensee states that there are no firearms stored in the home.
This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen, living room, front yard (fenced), backyard (fenced) and a detached garage. Areas accessible to children are: Living Room, the bathroom next to the kitchen and part of the backyard. Areas off limits: 3 bedrooms, 2nd restroom in the hallway, detached garage, front yard and other part of the backyard. Off limit rooms are kept locked. Per Licensee children are guided to the bathroom through the kitchen. There is no safety gate separating the off limits kitchen. Per Licensee, supervision is provided. LPA observed the kitchen cabinets to be latched. The backyard is divided by a wooden gate. Children play in the area right off the kitchen. The second half is off limits and it is kept locked. The Licensee's (4) dogs are kept in the off limits area of the backyard. Per Licensee, they do not enter the home during operating hours.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. There is telephone service via a cellphone. There is ventilation and heating. LPA observed A/C unit installed in the wall in the living room. There is a wall heater in the living room that is barricaded and inaccessible to children in care. Safe toys play equipment and materials were observed. Per the Licensee, the children play, eat and sleep in the living room. LPA observed children sleeping on mats. The on limits bathroom was observed to be safe and sanitary; no hazards were observed. Per Licensee she does not have any children enrolled that are under 12 months of age.

LPA observed the first aid kit in the living room. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children; detergents and cleaning compounds are stored in the latched cabinet under the sink in the kitchen which is off limits. Knives are kept in a container that is placed in a high shelf in the kitchen. Per Licensee medications are kept in a locked box in an off limits bedroom and there are no poisons in the home. Licensee stated there are no children enrolled who require medication. LPA inspected the fire extinguisher: The valve on the required 2A10BC fire extinguisher indicates fully charged and was serviced on 08/2020, as indicated on service tag. The Licensee states she provides food for children in care. Carbon monoxide and smoke detectors are installed in the living room. LPA did not have Licensee test the carbon monoxide and smoke detectors due to children napping at the time of the inspection. Licensee is required to test Carbon monoxide and smoke detectors monthly per Early Head Start program. LPA observed the monthly testing record; the last test occurred on 7/6/21. LPA will have Licensee send visual proof of working carbon monoxide and smoke detector at a later time.
Licensee states that she is not currently caring for infants under 12 months of age. Licensee states that if enrolled, infants under 24 months will sleep in the living room in a crib. Appropriate sleeping arrangements were observed. Cribs or play yard shall not hinder the entrance or exit from the sleeping space mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. LPA did not observe any infants swaddled while in care. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee.
The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 11/14/2022. The Staff #1 does not have current Pediatric CPR/ 1st aid certification; it expired in 01/2019. LPA advised Licensee that Staff #1 shall not be left alone with children if CPR/1st aid is not up to date. LPA advised that if a child shows signs of illness he/she/they shall be separated from other children. The licensee is observed to be operating within the license capacity limitations. LPA did not observe any children left in parked vehicles. Car seats shall only be used for transportation. LPA did not observe any children sleeping in car seats.

LPA observed that the Licensee has proof of the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file which was completed on 11/17/20. Assistant also had proof of the Mandated Reporter Child Care Training Certificate which was completed on 11/22/20. Licensee and Staff #1 have proof of immunizations on file.

All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 07/06/21.
LPA did not observe any pools, spas, hot tubs, fish ponds, or similar bodies of water during the inspection.
Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted.
LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.
LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices.
Incidental Medical Services (IMS):
Per licensee, there are no children enrolled that require IMS at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised Licensee how to access forms, regulations and quarterly updates online at: www.ccld.ca.gov. LPA also discussed the Provider Information Notices (PINS): Recently Approved Safe Sleep Regulations In Effect PIN 20-24-CCP, Benefits and Impacts of the Implementation of Guardian PIN 21-01CCLD. Capacity Handout (Small & Large) and LIC 9227 - Infant Sleep Plan and Guardian User access form were provided during this inspection.
At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.
Exit interview was conducted with Licensee, Karen Munoz. The Licensee was provided a copy of the appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.
A copy of the LIC 9213 was given to licensee—Notice of Site Visit.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
