25 | Licensing Program Analyst (LPA) R. Derraco conducted an unannounced annual inspection to the above facility on 07/20/21. LPA arrived at the facility at 10:30 AM and met with licensee, Elizabeth Lizarraga, who guided analyst on a tour of the facility. Also present during this inspection was co-licensee Angela Narvaez, Per Licensee, there are 11 children that are currently enrolled. There were five children present upon arrival. Three of the five children were above the age of 10 years old.
This is a one story house which is considered the main home and consists of 3 bedrooms, 1 bathroom, kitchen, dining room, living rooms, front yard, backyard (fenced), office, laundry room, and drive way. The main care area is located in the living room of the main house. A second one story home was observed in the backyard and made inaccessible by a wooden gate. Licensee states that the second home is occupied by her parents (S2 and S3). The second home was observed to have a bathroom, bedroom, dining area and kitchen. The off limit areas include the second one story home, two bedrooms in the main home, a driveway located at the side of the main home, and a back yard area. Door knobs leading to the off limit rooms were observed to have child proof covers. The main care area was observed to have age appropriate toys and materials. A window mounted air conditioner and a 2A10BC Fire Extinguisher were observed in the main care area with a service tag of 08/26/20. The dining area was observed to have a wall heater that is made inaccessible with a baby gate. Licensee states that S3 turned off the gas valve for the heater and it is inoperable. LPA advised licensee to keep wall heater inaccessible by using a gate or provide a tag from the gas company indicating that wall heater has been made inoperable. A carbon monoxide / smoke detector were observed to be working and mounted on the ceiling in the hallway. LPA observed a third bedroom being used as a napping room. Licensee states that children showing signs of illness will be separated from the group in the napping room. Age appropriate toys, a wall mounted television, a couch, a window mounted air conditioner, and a full size bed were observed in the room. The restroom that children use was observed to be safe and sanitary. A child proof lock was observed beneath the sink which contains cleaning supplies. A baby gate was observed Page 1 of 3 |
