Licensing Program Analyst (LPA) R. Derraco conducted an unannounced annual inspection to the above facility on 07/20/21. LPA arrived at the facility at 10:30 AM and met with licensee, Elizabeth Lizarraga, who guided analyst on a tour of the facility. Also present during this inspection was co-licensee Angela Narvaez, Per Licensee, there are 11 children that are currently enrolled. There were five children present upon arrival. Three of the five children were above the age of 10 years old.

This is a one story house which is considered the main home and consists of 3 bedrooms, 1 bathroom, kitchen, dining room, living rooms, front yard, backyard (fenced), office, laundry room, and drive way. The main care area is located in the living room of the main house. A second one story home was observed in the backyard and made inaccessible by a wooden gate. Licensee states that the second home is occupied by her parents (S2 and S3). The second home was observed to have a bathroom, bedroom, dining area and kitchen. The off limit areas include the second one story home, two bedrooms in the main home, a driveway located at the side of the main home, and a back yard area. Door knobs leading to the off limit rooms were observed to have child proof covers. The main care area was observed to have age appropriate toys and materials. A window mounted air conditioner and a 2A10BC Fire Extinguisher were observed in the main care area with a service tag of 08/26/20. The dining area was observed to have a wall heater that is made inaccessible with a baby gate. Licensee states that S3 turned off the gas valve for the heater and it is inoperable. LPA advised licensee to keep wall heater inaccessible by using a gate or provide a tag from the gas company indicating that wall heater has been made inoperable. A carbon monoxide / smoke detector were observed to be working and mounted on the ceiling in the hallway. LPA observed a third bedroom being used as a napping room. Licensee states that children showing signs of illness will be separated from the group in the napping room. Age appropriate toys, a wall mounted television, a couch, a window mounted air conditioner, and a full size bed were observed in the room. The restroom that children use was observed to be safe and sanitary. A child proof lock was observed beneath the sink which contains cleaning supplies. A baby gate was observed Page 1 of 3

at the entrance of the kitchen from the dining area. Licensee states children use the kitchen to pass through to the outside play area located on the side of the main home's driveway. LPA observed the outside play area to have safe toys, play equipment and materials. A deep freezer was observed in the outside play area. Licensee states that she will purchase a latch to keep freezer door secured and plans to store cold snacks for children in care during the summer. A wooden fence with a wooden gate were observed separating the outside play area from the drive way designated for parking vehicles. A dog was observed caged in the driveway. LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs.

Individuals who reside in the home were noted and discussed. All adults present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in the licensed childcare home. Licensee states that there are no firearms stored in the home.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety,


comfort, and cleanliness. There is telephone service via a cellphone that is used and the cellphone stays at the facility during operation hours.

Detergents, cleaning compounds, and other items which could pose a danger to children were observed to be locked beneath the sink in the kitchen. The licensee states that poisons in the home are stored in the locked garage. LPA observed the garage to have a pad lock and is inaccessible. No sharp objects were observed to be accessible in the kitchen area at time of inspection.

Safe sleep guidelines were discussed with licensee. LPA observed an Ingenuity playpen, model 10211, to have a fitted sheet and stored in the napping room. The playpen was observed to be free of loose articles and objects. No objects were observed to be hanging above or attached to the side of the play pen. LPA advised the licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each check shall be documented with the child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. Licensee states that she currently does not have any infants enrolled.

The licensee and other personnel have completed training on preventive health practices including Pediatric

First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 06/2023. LPAs observed that both Licensees have proof of the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file. File review was observed to have proper mandated immunization records. Children’s records were reviewed, including emergency information and were observed to be complete. A current children’s roster was available for review.

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted at the entrance of the home

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

Incidental Medical Services (IMS):
IMS was discussed with licensee. Per licensee, there are no children enrolled that require IMS at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised the licensee to access forms, regulations and quarterly updates online at: www.ccld.ca.gov. LPA also discussed the Provider Information Notices (PINS) on Recently Approved Safe Sleep Regulations in Effect: PIN 20-24-CCP, Updated Coronavirus 2019 Industry Guidance for child care settings - PIN 21-18-CCP (supersedes PIN 20-15-CCP, and Guardian User Account Access - PIN 20-20-CCLD.

At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.

Exit interview was conducted with Elizabeth Lizarraga, Licensee, including, but not limited to Appeal Procedures, Site Visit and Initial Appeal Rights.
