Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced case management visit on 11/12/21 to the above facility. LPA arrived at 12:35PM and was met by Sandra Conrad, Head Teacher, who guided analyst on a tour of the facility. There were 10 napping children and 4 staff present upon arrival. LPA was later met by LaShell Diggs, Program Director.

The purpose of the visit was to conduct a second follow-up for an incident that occurred on or before 09/3021 and was reported to the department on 10/04/21. The first follow-up visit was conducted on 10/22/21 which resulted with the facility being cited a Type B for not reporting in a timely manner. Information reported to the Department indicated that Staff #4 and #2 may have violated Child #1's personal rights.

LPA conducted interviews during the course of the investigation.

Based on the information obtained during the interviews disclosures were made that Child #1 was not comfortable in their environment due to Staff #4 and #2 possibly violating child's personal rights. The licensee was able to accommodate Parent #1 and Child #1 by transferring Child #1 to a different classroom making Child #1 more comfortable in a new environment.

LPA was not able to obtain sufficient information to determine whether Staff #4 and #2 indeed violated the child's personal rights. LPA did however advise on re-training staff on children's personal rights and appropriate re-direction.

The Notice of Site Visit must remain posted for 30 days. Exit interview was conducted with LaShell Diggs, Program Director, including, but not limited to Provider Rights, & Appeal Procedures.
