Licensing Program Analysts (LPAs) Austin Estrada and Jeanette Estrada conducted an unannounced required 1 year inspection at the above facility. LPAs met with Director Ytali Zavala who guided analysts on a tour of the facility. This is an infant program with a toddler component which consists of 2 classrooms. LPA observed the Toddler Program is conducted in an area physically separate from other child care components. Facility operation hours are Monday to Friday from 7:00 AM to 5:30 PM.
All areas identified on this report were inspected. Upon arrival, LPA observed six infants and two teachers in the Baby Biologists room (infant room) and nine toddlers and three teachers in the Little Explorers room (toddler room).
There are two preschool programs (license number 198020118 and 198400525) on the same premises. The following was observed during the tour of the facility: LPAs observed required licensing documents posted in a prominent, publicly accessible area which included, Facility License, LIC 610- Emergency Disaster Plan, Publication (PUB) 393- Notification of Parent Rights, PUB 269- Child Passenger Restraint System, LIC 613A- Notification of Personal Rights. LPAs observed LIC 9148 Earthquake Preparedness, Daily Activity Schedule, and Verification of Disaster and Fire Drills (last conducted in March 2023). Sign in/sign out sheets were reviewed.
LPA observed the infant classroom which is physically separate from the other child care components. In the Baby Biologists room, furniture and equipment were inspected for age appropriateness and good repair. Toys were observed to be safe, and do not have sharp points, edges, splinters and are not made of small parts that can pose a choking hazard. LPA did not observe any baby walkers or any items that fall into that category present in the facility. The classroom was inspected to ensure that the floors have a surface that is safe and clean. LPAs observed carpets and soft play mats on the floor of the classroom. LPAs observed cubbies available for children to store their belongings in the classroom. LPA observed six cribs in the napping area which was separated from the indoor activity space. Per Director, sheets are taken home by the parents and cleaned weekly. labeled and stored in the refrigerator if needed. LPA observed bottles to be labeled with infant’s names. LPA observed an infant in the crib sleeping with a blanket. LPA advised Staff #1 (S1) and Staff #2 (S2) that no loose articles can be in the crib while infants are sleeping. Staff immediately removed the blanket from the crib.
LPA observed the Little Explorers classroom (toddler classroom) to contain toys that were safe, and do not have sharp points, edges, splinters and are not made of small parts that can pose a choking hazard. The classroom was inspected to ensure that the floors have a surface that is safe and clean. LPAs observed carpets and soft play mats on the floor of the classroom. LPAs observed cubbies available for children to store their belongings in the classroom. LPA observed sleeping mats and sheets are taken home by the parents and washed weekly. LPAs observed a changing table an arm’s length away from a sink. Water is readily available via water pitcher. Per Director, toddlers are not “potty trained” at the facility. During the inspection LPA observed Staff #3 (S3) and staff #4 (S4) not to have the required education to provide care to children in the infant/toddler classes. Director then stepped in to replace S3 and S4 to be in teacher-child ratio. LPA advised Director of the educational requirements for teachers/ aides, which requires at least three college course units in infant/toddler development.
Both the infant and toddler classrooms contained functioning wall heater that was made inaccessible to children in care. LPA also observed air conditioning units in both rooms. The facility has completed the lead testing requirement. The facility is equipped with a centralized fire alarm system. LPAs observe a carbon monoxide detector at the time of inspection.
LPAs observed a fully charged Fire extinguisher. First Aid kit was observed and inventoried. Disinfectants and cleaning solutions were observed to be inaccessible to children in each classroom. Isolation area for an ill child waiting to be picked up is in the Director's office. LPAs observed medication to be stored in the Director’s office inaccessible to children in care. Per Director, the facility only administers prescribed medication if needed.
LPAs toured the designated infant and toddler outdoor play areas, which was observed to have age-appropriate toys and materials for children. The outdoor play areas were observed to be free of loose, sharp, and/or pointed parts. LPAs observed required cushioning under climbing structures to absorb a fall. Per Director, water containers are brought outside for children to drink while outdoors. LPAs observed adequate shade in the outdoor areas. LPAs did not observe any bodies of water at the facility.
Menus are posted online monthly via the pro care application. The facility provides two snacks for children in care. Children bring their own lunch which is properly stored or refrigerated. There is an option for an off site meal preparation that is not provided by the facility.

LPAs observed the kitchen/ food preparation area, and food storage area to be clean and free of litter, rubbish, rodents, and/or any other vermin. LPAs observed refrigerator to be clean and in operable condition.
Children’s records were reviewed for LIC 613A- Personal Rights, Admission Agreement, Immunization Records, LIC 700- Identification and Emergency Information, LIC 701- Physician’s Report, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 9227- Infant sleep form (0-12 months, and documentation of 15-minute Infant Sleep Check (0-24 months). All records were observed to be complete.
Staff records were reviewed for approved Pediatric First Aid and CPR certification, staff qualifications, LIC 508-Criminal Record Statement, LIC 503- Health Screening Report, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, and LIC 9108- Statement of Child and Mandated Reporter Training Certificate. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. LPA observed 2 of 5 staff, staff #4 (S4) and staff #6 (S6) not have a mandated reporter training certificate available for review. S4 did not have immunization records for measles and pertussis at the time of inspection. LPA observed S3 and S4 not to have the required education to provide care children in the infant/toddler classes.
During the inspection, children present was observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment.
Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Based on the LPA’s observations, interviews and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and Appeal Rights and report were reviewed with the Program Director Crystal Betancourt.
