At approximately 10:45 AM, Bardo Baluyot, Licensing Program Analyst (LPA) and Ana Chico, Licensing Program Manager (LPM), conducted an announced pre-licensing inspection. Met with Shabanna Syed, Director. All areas identified on the facility sketch were toured and indoor and outdoor measurements were taken. Applicant is requesting to be licensed for a pre-school for 30 children. A fire clearance was granted for 30 children without restrictions.

The proposed program will operate 8:30 AM - 3:30 PM, Monday - Friday. The classrooms and director's office are located on the second floor at the rear of the building which is primarily used as a religious facility. The ill/isolation area will be in the administrator's office (a cot will be available for children to use).

The adult restroom which is adjacent to the classrooms and administrator's office will also be used by children and is equipped with a child size sink and two child size toilets. Per director, step stools will be added to the two existing adult size sinks and toilets for children's use. A second adult restroom located on the same floor will be used for ill/isolated children. Step stools will also be added to the second restroom to accommodate children.

All classrooms were observed to be well ventilated, well equipped with plenty of natural lighting. Napping equipment will be washed weekly or as needed by staff. Flooring and rugs for the entire school are new.

There are fire extinguishers and a pull alarm system in place. The carbon monoxide detector is part of the hardwired fire system installed. Per applicant, children will be signed in upon entry to the classrooms.

Each room is equipped with a sink. A no touch pump dispenser attached to a five gallon water container is available as a drinking source. Per director, disposable cups will be provided once children are present so that they can drink freely and as they wish. All rooms are clean, well ventilated and well equipped.

Children will bring their own lunch and the school will provide all snacks. There is a refrigerator in the Administrator's office and a table is available to use as a prep area. Per director, dry snacks will be stored in adjacent storage closet, milk, cheese, juices and washed fruits will be stored in the fridge, fruits will be washed and cut in facility kitchen located on the first floor daily and disposable dishes and utensils will be used. Snack menus were posted and will be adjusted to meet federal food guidelines. Posting requirements were observed throughout the facility.

The outdoor play area is made up of brick wall and iron fence. Children will be escorted to and from the playground by staff at all times. A sectioned of open area inside the building (adjacent to the classrooms) will also be used as additional play space should the weather not allow for outdoor activities.

Per director, entire area will be cleaned and cleared of any debris and pictures of play equipment and shade structure will be sent once installed. Per director, water and cups will be brought out when children go outdoors. LPA has advised that utility pipes in play area be barricaded or padded for children's safety.

Per applicant, a waiver will be requested to use a staggered schedule as the outdoor space was measured to accommodate only 16 children.

The following items are pending prior to licensure: removal of identified furniture inside the classrooms, barricade/fencing by rear exit/stairs of second level additional play area, step stools for restrooms, updated menu with portion sizes and designated sink used for snacks, outdoor play waiver, padding/barricade for the utility pipes in play yard, cleaning of the yard and outdoor equipment in place outside.
