25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 04/29/2022. LPA arrived at the facility at 8:40 AM and met with Ramona Alexandi, Licensee who guided analyst on a tour of the facility. Also present during this inspection, was Sedik Hartoonian, Licensee’s Assistant. Per licensee, facility hours are Monday to Friday from 8:00 a.m. to 4:00 p.m. Per licensee, she is affiliated with Pacific Clinic Head Start Program.
This is a one story home which consists of 3 bedrooms, 2 bathrooms, kitchen, living room detached garage, front yard, and backyard (fenced). Per licensee the children use the living room, one bathroom adjacent to the living room, bathroom in the daycare bedroom, and backyard (fenced). Per licensee areas off limits to children and parents include: 2 bedrooms, kitchen, garage, front yard, and back of backyard. There is a pool in the off-limit area in the backyard which is covered by wood cover and pool is surrounded by walls in three sides and a gate in one side. Also there is an additional gate in the backyard to make the pool area off-limit and inaccessible to the children. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 9:10 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room (daycare classrooms), LPA observed children's table, chairs, and toys in the living room. LPA observed all electrical outlets were covered in living room. During this inspection there were 10 children present in the living room with licensee and her assistant and playing. LPA observed a safety gate was installed to the kitchen to make the kitchen inaccessible to the children. LPA observed off-limit areas bedrooms door where closed during this inspection. Next LPA toured the one daycare bedroom and did not observe any hazards. Next LPA toured the daycare bathroom and observed child proof safety latches on the cabinets where licensee stores sharp items and cleaning compounds. Per licensee, children only walk through the bedroom to go to the bathroom.REPORT CONTINUES ON NEXT PAGE 1 of 4 |
