On Thursday, October 22, 2020 at 5:30 PM, Licensing Program Analyst (LPA) Mayra Rivera conducted a pre-licensing inspection to increase capacity via teleconference. This pre-licensing inspection conducted by LPA Rivera, due to COVID 19 and precautionary measures the inspection was conducted via teleconference using FaceTime. The teleconference was conducted with Licensee Honida Koroma who guided LPA Rivera on a tour of the facility.

Family members residing in the home are spouse Nfagie Koroma, mother Odelia Badrie Deen, and two minor children Khalan Koroma and Galina Koroma who were also present during the pre-licensing inspection. The Case Management inspection is for the purpose of inspecting the facility is in regards to a capacity increase request. The Licensee is requesting to increase to a Large Family Child Care with a capacity of 12-14 children. LPA Rivera did not observed childcare children present at the time of the inspection. Operating hours will be Monday to Friday, 6:00 AM to 9:00 PM. The applicant is applying to care for children ages newborn to 12 years.

LPA Rivera observed and inspected the areas that are accessible to children and identified on the facility sketch: main living room (daycare area), 1 bathroom located in the hallway next to the family den and bedroom. LPA Rivera observed working toilet, paper towels, hand soap and running water. The front door is the main access entrance to daycare for drop off /pick up. LPA Rivera observed the backyard (fenced) for outdoor activity, and two end sides (left and right). One side has a safety gate in place to prevent children going into the side yard and the other side is the emergency outdoor exit. The gate door has a deadbolt lock to prevent children from opening the gate door. Incase of an emergency exit licensee is able to turn to deadbolt lock and open the door.

Areas off limits to children include: 4 bedrooms, 2 bathrooms, kitchen, garage and second floor. LPA Rivera observed a safety gate placed on the bottom of the staircase to prevent children going up the stairs and child proof knob balls on the door handles to prevent children from opening doors in off limit areas and to make areas inaccessible to children. LPA Rivera observed the fire place glass doors locked. LPA Rivera asked licensee to open the glass doors and doors did not open

Areas that will be used by the children were inspected for safety, comfort, cleanliness, telephone service, ventilation and heating (central). LPA Rivera observed Mr. Clean a cleaning compound and shampoo inside the bathroom sink drawer. The items were removed immediately and placed in the laundry room top shelf. Laundry room is inaccessible and has a child proof knob ball on the door handle. Laundry room is where licensee stores detergents, cleaning compounds and hazardous items that can pose a danger to children. Knives and sharp objects are placed in the kitchen drawers and kitchen is an off limit area. LPA Rivera observed the kitchen door closed and door handle with a child proof knob ball in place LPA Rivera observed child proof knob balls in place on the door handles to prevent and make off limits areas inaccessible to children. LPA Rivera did not observe poisons in the home and licensee stated that there are no poisons in the home. The licensee was advised that if any poisons are purchased, it is required to be locked with a key or combination lock. Licensee was advised that if food is brought from the children’s home, all containers must be labeled with the child’s name and properly stored or refrigerated.



LPA Rivera did not observe pets, firearms, weapons or bodies of water on the premises. Per applicant, there is no firearms, weapons or bodies of water on the premises. LPA Rivera observed age appropriate toys and napping equipment for the children. LPA Rivera observed electrical outlets covered in accessible areas, fire extinguisher located in the daycare room and the valve on the required 2A10BC fire extinguisher indicates fully charged and serviced on March 25, 2020. LPA Rivera observed the smoke and carbon monoxide detector in the daycare room and LPA Rivera asked licensee to press the smoke and carbon monoxide detector button and LPA Rivera heard the sound of operable detector. The smoke and carbon monoxide detectors are a combo in one. First Aid kits are available in daycare room.

LPA Rivera observed proof of pediatric First Aid/CPR certification (expires 11/2020).The applicant completed the preventative health and safety training on September 27, 2020 and mandated reporter (AB 1207) training on February 22, 2020. The applicant was advised that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com.

On Monday, August 24, 2020 at 5:30 pm, LPA Rivera conducted a RAST call to go over information in regards procedures and information related to Covid 19. Information covered are as follows: enhanced sanitation practices, physical distancing, use of face coverings, screen health practices, hand washing procedures, covid 19 specific exclusion criteria, and pick up and drop off procedures,

Note: A qualified assistant must be present and actively involved in caring for children whenever nine (9) or more children are present at the facility. This includes outdoor activity and activity away from the child care facility. Note: LPA discussed with Licensee the possible capacity and ratio scenarios approved under Title 22. LPA also reminded Licensee of the following:

1. All adults working with the children, residing in the home or having initial presence in the facility (during operating hours) must be fingerprint and TB cleared.



2. The capacity and ratio requirements for a small family child care home resume when no assistants are present.

3. All medications, cleaners, chemicals and hazardous items (such as knives, sharp objects etc.) must be inaccessible.

4. The following are not allowed in a family child care home: Baby bouncers, saucer chairs, johnny jumpers, baby walkers, car seats, rockers and trampolines as they may be an immediate risk to children in care.

Note: Licensee has an approved Fire Clearance dated August 12, 2020 for 14 children. .

Based on LPA Rivera observations and inspection, the Licensee is approved for a Large Family Child Care Home as of October 22, 2020. The Licensee must adhere to terms and limitations on the license.

Exit interview was conducted with Licensee, via tele-inspection, during which appeal rights were explained. This report along with a copy of the Notice of Site Visit and appeal rights will be sent to licensee via email (honidakhalan@gmail.com) with a read receipt or confirmation of receipt of email, which will act as licensee signature.
