25 | Licensing Program Analyst (LPA) A. Lucero conducted an Announced Pre-Licensing Inspection to inspect the facility for initial licensure. Upon arrival, LPA met with Applicant Alexis Moser who guided LPA on an indoor and outdoor tour of the facility. There were no day care children present. The facility is a four bedroom and two bathroom home. Present at the time of inspection was licensee's father (individual #2) who is fingerprint cleared.
Individuals residing in the home are the applicant, husband, licensee's mother and father, and two minors. The Applicant has all appropriate forms posted. Applicant’s CPR card is valid through November 2019. LPA confirmed with Applicant that all adults residing/working in the home have obtained criminal record/TB clearances.
There is an operational smoke and carbon monoxide detector and required 2-A10BC fire extinguisher maintained in the home. The home has electrical outlet covers throughout and maintains a First Aid Kit. There are some age appropriate materials available for children to be enrolled in care.
Per Applicant, there are no firearms, ammunition, or weapons present on the premises. Furthermore, there are no bodies of water or spas on the premises. The outdoor play area is a fenced backyard, which is free of hazards. Applicant is applying for a capacity of 14 children, the department received a granted Fire Clearance on 04/03/2019. LPA observed a fireplace that has a secured children's gate to prevent access.
The Applicant was advised she shall be present in the home and shall ensure children in care are visually supervised at all times. Applicant also advised once Licensure is granted, when temporarily absent from the home, Licensee shall arrange for a criminally record cleared substitute adult to care for and supervise children in her absence. Applicant advised children shall not be left in parked vehicles at any time.
Per Applicant, the following areas will be used for day-care: Living room, dining area, one bathroom located in hallway, back yard.
