Licensing Program Analysts (LPA) Elka Chavez conducted an unannounced case management inspection to ensure that the Type B deficiencies cited on 08/04/2018 have been cleared. LPA met with Aida Ramos, Licensee, who guided analyst on a tour of the facility. There were 4 children present during this inspection.
LPA did not observe the baby bouncer child #2 was observed to have fallen asleep in on 8/04/21. At 2:25pm LPA observed child #2 sleeping in a baby bouncer. LPA observed child's mother remove the baby bouncer from the day care.
LPA reviewed and received documentation requested to clear citations. LPA cleared deficiencies on this date and provided a copy of the Licensing Report to Aida Ramos, Licensee. LPA issued POC clearance letter during the inspection.
Based on the LPA’s observations and interviews, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
----A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.
Exit interview was conducted with Aida Ramos, Licensee, including, but not limited to Appeal Procedures and Appeal Rights.
