An announced Pre-licensing/change of ownership inspection was conducted on this date by Licensing Program Analyst (LPA) B. Emiko Bell in order to take measurements of the indoors and of the outdoor play space of a pre-school. The application was received by the Department on 04/12/19 but was not assigned to LPA Bell until 05/08/19 (LPA Bell reviewing for LPA Eivazian.)

Upon arrival, LPA was greeted and let into the facility by Applicant Zarui Yerkanyan, to whom the reason for the inspection was announced and who guided LPA on a tour of the pre-school classrooms and of the pre-school playground. The Center is a two-story residence located on the corner of Cedar and Lexington which is currently licensed under license number 198002423. (Applicant provided LPA with a copy of the current "Zoning Use Certificate," which approves that the residence be used as a "Private School.")

Census: There were 4 children with 1 staff in the classroom. Staff-child ratio was met.

The Center will operate year-round. The proposed days and hours of operation are Monday-Friday 8:00 A.M. to 6:00 P.M. The Center proposes to serve children who are ages 2 to 5.

There is age-appropriate furniture and equipment in the classroom. The playground equipment is stored on the playground.

There is phone service. There is one cordless phone and the Owner has her personal cell phone.

The classroom also has one children's bathroom which consists of a child-size toilet, an adult-size toilet and two regular-size sinks. Applicant stated that she will purchase a changing table with the understanding that a sink has to be in arm's-reach length while a child is being changed.

Incidental Medical Services: Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The Center has central heating and air-conditioning. Overhead lighting provides the majority of the light in the classroom, but natural lighting comes through the windows as well.

Based on measurements taken on this date, the maximum indoor capacity shall be for 30 children. The maximum outdoor capacity is for children. Applicant is applying for 30 children.

Fire clearance was granted on 05/30/19 for a capacity of 30 children.

An exit interview has been conducted with and a copy of this report has been signed by and provided to Applicant Zarui Yerkanyan. Appeal Rights have been provided and explained to same.

All posting requirements except of the Daily Schedule were observed on the Parent Board, which is posted on the wall directly to the left of the entryway. The Daily Schedule is kept outside in a shadow box above the sign in/out sheet.

The sign in/out sheet is kept outside during daycare hours.

There is a monthly menu posted. The Center will only serve A.M. and P.M. snack. A.M. snack will be served at 10:30 A.M. and P.M. snack will be served at 3:00 P.M. Drinking water will be dispensed from a portable water dispenser with individual, disposable cups or the child's sippy cup. The snacks are stored in a cabinet by the exit to the playground. There is a mini refrigerator outdoors.

There is one First Aid Kit kept outdoors and one in the classroom. Both were inventoried during today's inspection. The Emergency Backpack will be stored in the closet where the children's cubbies for bedding is kept. The Center also will provide each child with an "Emergency Kit," which has a three-day supply of food, a light, etc.

When a child becomes ill during school hours, the Center will call their parent. The designated isolation area will be located in the corner across from the adult/staff restroom, as a cot can be placed there and the child would have easy access to the adult/staff restroom.

Cleaning supplies will be kept in the closet which is directly across from the closet in which the children's cubbies which stores their bedding is located; it is locked by key.

Children will nap on cots. The cots are stored in the adult/staff restroom.

There is a fire alarm system. There is a carbon monoxide detector located next to the dumbwaiter. There are two fire extinguishers, one by the the entryway and one by the exit to the playground. Both fire extinguishers were serviced 07/09/19 and both are Type A Size II, Type B:C Size I.

The outdoor activity area is surrounded by a cement wall. There are two wrought iron gates, one of which is kept locked by key and the other of which is kept locked by a padlock. The garage is being used for storage and is locked by three padlocks. There is asphalt, cement, sand, wood chips and rubber foam cushioning. The sand is contained in a sandbox. The wood chips are under the tree/bushes in two separate areas. The rubber foam cushioning is under the play equipment. The cement is around the rubber foam cushioning and under the tables and the rug in the shaded area under the covered patio. There is a door from the room which leads directly to the playground. There is a drinking faucet and a sink on the playground. There is age-appropriate play equipment.

The Center will not offer transportation.

The encumbered space in the classroom is the closet where the children's cubbies in which they will store their bedding, the wall on which the dumbwaiter is and the wall on which the air-conditioning vent is located. The cubbies in which the children will store their personal belongings will be kept outdoors by the sign in/out sheet during daycare hours. There is no encumbered space on the playground.
