Licensing Program Analysts (LPAs) Thelma Razo and Lissete Gonzalez conducted an unannounced Required – 1 Year inspection on 6/30/2021 at 9:30AM. LPAs met with Lead Teacher Cristina Angelo who guided analysts on a tour of the facility. This is a preschool program which consists of 8 classrooms and 1 multi-purpose room: Rm 101, Rm 102, Rm 109, Rm 110, Rm 115, Rm 117, Rm 118. Rm 140, and Rm 114 (which will not be used as a classroom but as a multi-purpose room). This program will serve children ages, 2.5 years to 5 years and will operate from Monday through Friday 7:00 AM - 6:00 PM.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room 110 are outside in the play area: 2 Staff with 10 preschoolers; Room 117: 2 Staff with 10 preschoolers; and Room 118: 2 Staff with 7 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

PHYSICAL PLANT
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly or as needed. There are laundry services available on site and custodian launders linens twice per week or as needed. Napping equipment (cots/mats) were observed in each classroom. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Lead Teacher states there are no poisons at the facility. Carbon monoxide detectors were observed and are operable.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to me clean and safe. Al materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

FACILITY RECORDS
There is at least one person trained in CPR and Pediatric First Aid present during this inspection. (C-VUSD -All staff have received an active criminal record clearance as a condition of their employment with the Covina-Valley Unified School District.)
Children’s Records were reviewed. (Name, address, telephone of child’s authorized representative, Medical Assessment for completeness; Inspection of required forms was made and documented on the LIC 857.
LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza. All staff have been given on-the-job training sanitation principles, housekeeping, including universal health precautions.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 6/21/2021.

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and

appropriateness to children in care. The facility provides breakfast, lunch and a PM snack. Meals are delivered daily by Covina Valley Unified School District located at another school site. Meals are individually served to children in care using disposable containers and utensils.

First Aid supplies were observed in each classroom. According to the Lead Teacher, medication is only administered to a child when accompanied with a doctor's note and is stored in the classroom. The facility was observed to be equipped with an isolation area for any child who becomes ill and is located in Room 135 which is the nurse' room.

Medication: This facility provides Incidental Medical Services – IMS. LPAs reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. UPDATE: LPA advised Facility Representative of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, Guardian User Account Access - PIN 20-20-CCLD and LIC 20-24-CCP Recently Approved Safe Sleep Regulations in Effect to include Safe Sleep Frequently Asked Questions.
The Notice of Site Visit (LIC 9213) was provided – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

There are no citations being issued at the time of inspection under California Code of Regulations, Title 22 Division 12.
