Licensing Program Analyst (LPA) Judy Mora met with Applicant Dominic Armendariz for the purpose of conducting a Pre-Licensing Inspection. The Applicant is applying for a Preschool License. Applicant is requesting a total capacity of 24 children. The proposed program will operate 2 half day sessions Monday through Friday with the first session from 8:15AM - 11:45AM and 12:30PM - 4:00 PM. The proposed program is located in The Puente Learning Center, where there is a charter school grades TK-8. No other license is held by applicant/licensee. A fire clearance was granted for 24 children.

All indoor and outdoor areas identified on the facility sketch were toured during this visit. The proposed preschool program will operate in Room 150. Per applicant, the program will enter through the main double door entrance. In order for the children to get into the classrooms hallway, scanned badge access is required. Per Director, Teachers will be greeting parents at the double doors in order to walk children to their classroom. Sign in/out procedures will be implemented outside of the second double doors. Parent board with required postings was observed.

The applicant states that the director's office (Book Room) will be used as the ill isolation area and children will use the staff restroom. The facility will provide snacks due to the half day program. There is a first aid kit in the classroom. Carbon monoxide detectors and smoke detectors are present in the building. There is a water fountain for drinking inside the classroom.

LPA observed sufficient storage space for children’s belongings. LPA did not observe containers used to discard food. Cleaning compounds and any poisons are stored in an area that is off limits to children.

Once the above items have been corrected, and all application documents approved, the department will make its determination on whether or not to approve the Applicant for a preschool license.

THE ABOVE CORRECTIONS ARE DUE ON OR BEFORE 11/22/19, if additional time is needed LPA will be notified.

The following was discussed with the Applicant:

Mandatory Forms for the children’s files and staff files. Requirements for fire drills, earthquake drills and documentation on a quarterly basis for both were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Applicant was advised how to access forms and Regulations online at www.ccld.ca.gov. Applicant was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. The Applicant was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

LPA advised Applicant that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. LPA informed Applicant to log onto web site www.ccld.ca.gov to obtain forms and LIVE SCAN application. Applicant advised that records must be updated with the required documents immediately, and records for all children and staff must be maintained for three (3) years after separation from the facility.

The Applicant was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to Community Care Licensing (CCL) within the time frame specified by the regulation. Applicant advised to visit www.shotsforschool.org for immunization information.

The LIC 9213, Notification of a Site Visit was posted. This notice is to be posted for 30 consecutive days. Failure to keep this notice posted for the 30 consecutive days will result in an immediate $100 civil penalty.
Exit interview conducted with Applicant, and a copy of report was given. Appeal rights were issued and discussed.

The playground is located in the front area of the Puente Learning Center building. During this inspection, LPA was notified that in order for the preschool children to get to the preschool playground they would have to go through the Kinder Room (Room 140). LPA advised Director that the preschool children will not be able to cross through the Kinder room and another route will be needed prior to being licensed. The surface of the outdoor activity space was observed to be in a safe condition and free of hazards, however, LPA observed that the playground has not yet been set up with equipment. LPA did not observe water available outdoors, however, Director stated that a water jug and disposable cups will be taken outdoors. No shade was observed on the outdoor playground. The applicant states that there are no bodies of water on the premises and none were observed during this visit.

Incidental Medical Services – IMS was discussed. The facility will not administer any medications. LPA observed two toilets and three sinks. LPA measured the indoors and outdoors as follows:

Room 150 allows for 21 children indoors.

Outdoor Play yard allows for 25 children outdoors.

The following requirements are needed prior to the applicant obtaining a preschool license:

1. Drinking water needs to be available outdoors.
2. Outdoor yard needs to be setup with equipment.
3. Shade structure is needed for outdoor space.
4. Trash can with tight fitting lid is needed.
5. A new route for preschool room to the yard is needed.
