REQUIRED- 1 YEAR INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 08/30/23. LPA arrived at the facility at 10:20 AM and met with Arman Igitkhanyan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, were Ovsanna Mamyan, Licensee's wife/assistant and Syuzanna Vardanyan, Licensee’s Assistant. Per licensee, Arman Igitkhanyan facility hours are Monday to Friday from 7:00 a.m. to 11:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee. At 11:10 a.m. Lusine Nalbandyan, licensee's assistant arrived to the facility. At 11:15 AM, licensee, Arman Igitkhanyan left the facility to take his child to a doctor appointment.

This is a one story home located on the first level. That home consists of 3 bedrooms, 2 and 1/2 restrooms, dining room (classroom), living room (classroom), kitchen, laundry room, backyard (fenced), detached garage and front yard.

Per licensee the children use the living room and dining room (classrooms), 1/2 bathroom adjacent to the kitchen, kitchen, 1 bedroom next to the kitchen and front part of backyard (fenced). Per licensee areas off limits to children and parents include: Two bedrooms, two bathrooms, back of backyard, front yard, detached garage (storage), and laundry room. LPA did not observe stairs in the home during this inspection. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 10:30 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms), LPA observed sofas, a coffee table, and TV in the living room. LPA observed children's table, chairs, and toys in the dining room. LPA observed all electrical outlets were covered in living room and dining room. There is a fireplace in the living room which has been blocked of with big sofas to prevent access. LPA observed 9 children, 2 being

infants in the dining room playing with licensee's two assistants. Next LPA toured the two off-limit bedrooms and two off-limit bathroom in the hallway and did not observed any hazards. LPA observed off-limit areas doors were closed during this inspection. LPA observed plastic door knob on off-limit areas doors. LPA observed licensee's family members personal belongings in the home. Next LPA toured the kitchen and observed child proof safety latches on the cabinets where licensee stores sharp items and cleaning compounds. LPA observed a sliding safety gate was installed to the kitchen. Next LPA toured the half bathroom adjacent to the kitchen and did not observe any hazards. Next LPA toured the 1 day care bedroom and observed children toys and napping cots in the bedroom. LPA observed the required 2A 10BC fire extinguisher was anchored to the wall by the half bathroom. It was serviced on 09/22/22 as read by LPA from the service tag. Licensee tested the carbon monoxide and smoke detectors in the living room. It sounded off the alarms and heard by LPA to be functional.

Next LPA toured the backyard fenced. Per licensee children only use the front part of the backyard fenced that is separated from the rest of the backyard by 6 feet high vinyl walls, also the daycare outdoor playarea is fenced by 3 feet high fences. The outdoor playarea ground is covered with artificial grass, LPA observed play structures, play houses and toys in the backyard. Next LPA toured the back of the backyard. LPA observed the gate swing away from the back of backyard. LPA observed an empty jacuzzi in the back of back yard, off-limit area which was covered and side latches are locked. The jacuzzi cover stands an adult weight. Next LPA toured the off-limit detached garage (storage) and observed licensee's personal belongings in the garage. Per licensee, parents enter the facility through the main entrance door which leads to the living room.

Per licensee, they provide food for children in care. Licensee was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated. Per Licensee's wife, Ovsanna Mamyan at 11:00 a.m., there are weapons and firearms in the premises. LPA observed the fire arms are stored in off-limit area and firing pins were removed. Ammunition were locked separately in off-limit area. Per licensee's wife there are no pets on the premises.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 09/2023, Licensee's wife, Ovsanna Mamyan Pediatric First Aid and CPR expires on 07/24. Licensee, Arman Igitkhanyan completed required mandated reporter training on 10/02/21. There are first aid supplies available on the premises in the daycare bathroom. LPA advised licensee that if a child shows signs of illness he/she/they shall be separated from other children.

The licensee was observed to be operating within the licensed capacity and is not exceeding the required limitations during this inspection. During this inspection 9 children were present in the home with licensee and his three assistants. All adults present have obtained a criminal record clearance on Licensing Information System (LIS). Per licensee, currently he has 3 assistants. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

At 11:30 a.m., facility roster was reviewed and completed. Per licensee currently there are 14 children enrolled. LPA reviewed 5 children's records, including emergency information from 14 enrolled children, and all 5 reviewed files were completed. The licensee and his three assistants have proof of immunization against influenza, pertussis, and measles. All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 07/06/23.

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted at the wall by the entrance. LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility.

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee's wife, licensee smokes but not in the daycare area. There is telephone service via a cellphone that is used and the cellphone stays at the facility during operation hours. Per licensee she does not carry liability insurance or a bond in accordance with standard established by Family Child Care statue. Signed statements (LIC282) on 5 reviewed files.

INFANT CARE: Licensee's wife, Ovsanna Mamyan states that they provide care to infants. Currently there are 3 infants enrolled in the daycare who are 12 months older. Per licensee's wife, infants sleep in the living room and daycare bedroom where they are constantly supervised. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee. LPA provided the licensee with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices.

SAFE SLEEP: LPA discussed the safe sleep regulations with licensee, and discussed the Child Care Licensing Safe Sleep webpage at http://www.cdss.ca.gov/inforescources/child-care-licensing/public-information-and-resoucrces/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at http://www/cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Medication: Incidental Medical Services (IMS) policy was discussed. The licensee states that she will provide IMS. Per licensee, there are no children enrolled that require IMS at this time. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised the licensee to access forms, regulations and quarterly updates online at: www.ccld.ca.gov.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today.

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Licensee's wife, Ovsanna Mamyan was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee's wife, Ovsanna Mamyan, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
