A pre-licensing inspection was conducted today by Licensing Program Analyst (LPA), Alanna Gontarek and Licensing Program Manager (LPM), Claudia Guangorena, to inspect and measure this facility for capacity determination. Licensing staff met with applicant/owner, Minerva Carrion, along with Director, Kathy Beaubien who assisted Licensing staff with touring the facility.

This is a proposed Pre-School Program which consists of 2 classrooms; Pre- K Room and 3- Year Old Room. This program will serve children ages, 2 years to 6 years. This program will operate Monday - Friday 7:00 AM - 6:00 PM. All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken.

Sign-In and Sign-Out forms are located in each classroom and a health check will be conducted prior to entering the classroom on a daily basis. Applicant states that she will place tables outside each classroom to have parents sign in, using individual small pencils, and take temperatures of each child upon entry into the classroom due to COVID-19 health crisis. There is drinking water readily available in each classroom. Licensing staff observed drinking containers and disposable cups in both Pre-K and 3 Year Old classrooms. Applicant states she will be installing drinking faucets in each classrooms' sink, and will be temporarily using a pitcher in the Pre-K classroom and a pitcher with a spout in the 3-Year Old classroom. Licensing staff advised Applicant to replace heavy container (in the Pre-K room) with the same water jug with a spout that is used in the 3-Year Old room.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens for napping will be washed on site weekly.

Napping equipment (cots) were observed in a separate storage room. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. Licensing staff observed that facility does have a carbon monoxide detector, smoke detector, and a serviced fire extinguisher. Fire extinguisher was last serviced on 1/6/2020. Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible to children and stored in a locked closet, on a high shelf. Applicant states that all poisons are locked in a locked janitor's closet with a combination lock.

This program will provide an AM snack and a PM snack. Licensing staff advised staff to have a snack containing 2 different food groups. Parents are required to bring their child(/ren) lunch. A monthly snack menu is available and posted.

All kitchen areas/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Licensing staff advised applicant and Director that all boxes of food items need to have the expiration date labeled on the box, and all opened containers need to be placed in a Ziploc bag, and expiration labeled on the item as well. Trash cans, used to discard food, have tight fitting lids. The facility was observed to be free of flies, other insects and rodents.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. Licensing staff advised applicant and Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The surface of the outdoor activity space is in a safe condition and is free of hazards. Licensing staff observed the astro turf to not be leveled or secured to the ground. Applicant states that the school will be receiving a tape that will connect the pieces of turf , and will be secured to the ground. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Applicant states they are expecting additional shade material for play ground. Licensing staff observed a tarp used for shade. Availability of outdoor drinking water was observed. Per applicant, a sign, indicating boy's and girl's bathroom, will be placed on the door. There are no bodies of water on the premises.

Applicants states that medication will be stored in the office in a locked box. First aid kits are available in each classroom and were observed to contain the required first-aid supplies.

The Preschool office room will be used as an ill/isolation area. Ill children will use the staff restroom across from this room. Also there is a cot available for ill child to rest if needed. This room was observed to be conducive for children's use at this time.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Items discussed during this visit:


I. Licensing does not tolerate the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both. II. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
III. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
IV Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present (opening and closing of the day).
V. Designated Staff: The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
VI. Qualifications: Educational background, training, and/or experience for each staff present must be available for review. Per applicant, Teacher’s will have a copy of their teacher permit available.

VII. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
VIII. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.
IX. LIC 311A - Records to be Maintained at the Facility: Child Care Centers was provided to the applicant during this visit.
X. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

Licensing staff advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.

Prior to Licensing the following items are to be corrected and in compliance with Title 22, California Code of Regulations:



1. Applicant needs to place a sign on the door of the Preschool office stating, "Preschool Office."
2. Applicant states the school will tape the pieces of turf, connecting and leveling the pieces, securing turf to the ground.

The above items are due on or before June 19th, 2019

The applicant is seeking to provide care for 48 preschool children; however, after final review of application and based on measurements taken today, the maximum allowed will be 46 preschoolers. A fire clearance has been granted. Exit interview conducted with Applicant, Minerva Carrion and Director, Kathy Beaubien. Appeal rights were issued and discussed.
