Licensing Program Analyst (LPA) Judy Mora conducted an unannounced Required inspection. LPA met with April Camacho who was in the front office. LPA then met with Assistant Director, Ian Spik. This is a preschool age program licensed for 96 children which operates Monday – Friday from 7:00 AM – 5:00 PM. Director, Cristina Spik was not available for this inspection. Per the Director there are 44 children enrolled. LPA's temperature was checked upon arrival by Ms. Camacho.

At approximately 10:15 AM LPA Mora began facility tour with Assistant Director. All areas identified on the facility sketch were inspected. LPA observed 08 children with 02 staff in the Yellow Room, 09 children with 01 teacher in the Green Room, 07 children with 01 staff in the Red Room and 10 children with 01 staff in the blue room. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The facility was observed to be within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, there is a classroom designated as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menu was observed, however, Director indicated via telephone that children are currently bring their own snacks and lunch for summer. LPA advised Assistant Director that snacks are required to be provided by the center for full day programs. A technical advisory was provided. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin.

There is drinking water available in all indoor classrooms and a water fountain outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. The licensee takes measures to keep the facility free of flies, other insects and rodents.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Areas around and/or under climbing equipment and slides have rubber cushioning material to absorb a fall. Assistant Director states there are no weapons or firearms on the premises.

Sign in and out sheets were reviewed at approximately 10:05 AM, to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPA observed 34 children present and 32 children signed in. This is a potential risk to the health and safety of children in care. Staff Records were reviewed to ensure that a health screening report is on file. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.



SB792 (Immunization Requirements for Staff and Employees) was discussed. All staff have required immunization's on file.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Incidental Medical Services (IMS) policy was discussed. Per Assistant Director, there are no children on medication currently. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.

The following was discussed:

· Reporting Requirements.


· Individuals who are 18 years of age or older living in the home must obtain a criminal record clearance. Individuals within one month of their 18th birthday must be fingerprinted immediately. Failure to obtain a criminal record background check clearances prior to initial presence in the home will result in an immediate $100.00 dollar or more per day Civil Penalty.
· The facility shall ensure that each child is accorded the following personal rights: to be accorded safe, healthful and comfortable accommodations.
· All personnel shall be in good health and shall be physically and mentally capable of performing assigned tasks; personnel that pose a threat to the health and safety of children shall be relieved of their duties.
· All personnel shall be given on the job training in housekeeping, sanitation principles and universal health precautions.
LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

The deficiency listed on the following page was observed by the LPA and is being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiency that is being cited and needs to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
