Licensing Program Analyst (LPA) Cynthia Reyes, conducted an announced pre-licensing site Inspection to inspect and measure the requested preschool component for capacity determination. LPA met D' Arezzo , Director. Due to COVID- 19 precautionary measures were taken during the entire inspection and all individuals present during this inspection wore appropriate personal protective equipment. The applicant is requesting a capacity of 42 preschool children (2-5 years old). Days & Hours for the proposed preschool will be Monday- Friday 6:30 AM-6 PM.

At 3:25 PM, LPA began the tour of the facility with the Director Liane, beginning at the entrance to the preschool black wrought iron gate leading from the parking lot located at the back of the church. Director states parents will enter and exit the black gate and walk their child to their class room and sign in/out sheets for parents are located on top of the child's cubbies located right outside each of the class room doors. Staff will check parents and children temperatures at the front door of the class room, hand sanitizer is available for use at the front gate. No parent will enter the class rooms at this time, unless they request to enter, where at that time all precautionary measures will be taken for that individual entering, for the children in care and the staff.

The Preschool office where the Directors office is located is located next to class room 1 and will be used as the children isolation area with a fully equipped first aid kit and cot/mat for resting. The staff rest room is located right out side the Directors office inside the church side door, which will also be used by the ill child. LPA observed in the church an adult men and woman's bathroom. the staff rest room is the designated men's bath room (Adults only) and the women's bathroom is for the preschool children only (4 toilets and 2 sinks).

The tour continued Next door to the directors office where class room 1 is located. Each preschool class room has labels designating rooms 1,2,3,4, and 7, as rooms used by the preschool. Rooms 5 and 6 are not used for the preschool program. LPA and Director measured each class room together to determine capacity for each room and for the total capacity requested.

LPA observed in Class Room 1 to have (1 toilets and 1 sink), Room 2 (1 sink), Room 3 (1 sink), Between class room 2 & 3 is (1 toilet), Room 4 and 7 (No toilets or sink), Bath room in church has (4 toilets and 2 sinks). The Total: 6 Toilets X 15 = 90 and 5 Sinks X 15 = 75 Preschool children. All toilets and sinks are safe, sanitary and are in operating conditions.

3:50 PM LPA observed In each class room, Indoor play equipment, furniture and materials for preschool children were inspected by LPA to be clean, safe, age appropriate and in good repair. Cleanliness and safety of the facility was also observed. Licensing staff observed children sleeping mats are located in each class room. Children's parents will bring sheets/blankets each week that will be located in each of their own cubbies. Linens will be taken home every Friday by child’s authorized representative for laundering. LPA observed small throw away cups and water jugs available in each class room. Director states has extra linen if needed.

Telephone service, heating, lighting and ventilation were evaluated. LPA observed there is fire extinguishers, smoke detectors and a carbon monoxide detector on site located throughout the facility that meets statutory requirements. No disinfectants, cleaning solutions, poisons and other hazardous items that are dangerous to children were observed to be accessible. There is a fully equipped first aid kit available in each of the classroom and were observed to contain the required first-aid supplies. If any lifesaving medication are needed they will be stored in a locked cabinet in the directors office.

This facility will provide AM and PM snacks and children will bring their own Lunch. Snack and extra lunch will be in the kitchen on site and that is clean, safe and in good repair. A monthly snack menu is available and posted. LPA advised the licensee to label and date all food, dishes and bottles that are brought from home for proper storage. No transportation is being conducted by the facility. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

LPA observed the facility was not adhering to local public health guidelines regarding markers for social distancing and one of the plan of corrections given to the Director is to place markers for 3/6 feet social distancing on table and floors and instructed that Covid posting should be placed in each class room at eye level for the children. Mask are to be worn by all children age 2 and up at this time.

At 4:25 PM, LPA toured the outdoor activity area and observed play equipment and toys to be age appropriate, safe, sanitary and in good repair. Shading was observed to be provided by large trees, by the facility itself for the side yard and the covered cement area. All areas around or under high climbing equipment were observed to have cushion material to absorb a fall. Outdoor play yard Measurements were taken for a total capacity of 153 preschool children.

Items discussed during this Inspection:

Licensing does not tolerate the following: Refused Entry to a Facility or Any Part of a Facility is a


violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
Current Children’s Roster LIC 9040: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This children roster must be updated and available to the licensing agency upon request.
Pediatric First Aid and CPR: At least one person in the facility, along with the Director/Administrator must be trained in Pediatric First Aid and CPR and present in the facility.
Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
Qualifications and staff files: Educational background, training, and/or experience and all required documents by the department for each staff present must be available for review.
Children’s Records: All required licensing documents must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Director advised to visit www.shotsforschool.org for immunization information.
Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or
volunteer at a day care center if he or she has not been immunized against influenza, pertussis, & measles

Criminal background clearance: LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.
Reporting Requirements Section 101212: Changes at the facility must be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

4:50 PM, Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

LPA advised the Applicant to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

LPA observe the parent board is located outside the preschool office to have all the required licensing forms and Covid posters accessible for parents and all to view, which included the Parent’s Rights Poster (with the complaint hot line telephone number).

An approved fire clearance was granted on 06/16/2021 for capacity of 42 preschool children.

5:10 PM LPA also consulted with the applicant regarding COVID-19 health and safety guidelines on this date. Licensing staff observed COVID- 19 postings posted in different areas of the day care on this date. During this inspection LPA also discussed PIN 20-06 CCP, Social and Physical Distancing Guidance and Healthy Practices for Child Care Facilities in Response to The Global Corona virus (COVID-19) Pandemic Written in Collaboration with The California Department of Education. LPA reviewed with the applicant the Child Care Covid-19 Self-Assessment guide that the applicant will fill out and email to the LPA by 04/17/2021.

Based on the LPA observation, the following correction need to be made prior to submitting application for final approval.



- 3/6 Ft. markers on the floor and tables in each class room
-Copy of Lead certificate
-Final File review.

Based on LPA measurements taken on this date, the capacity will be approved for the requested 42 preschool.

The application will be submitted to be licensed after completion of correction needed. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license

5:20 PM. An exit interview was conducted with Director Liane D' Arezzo, and a copy of this report has been signed by and given as well as Appeal Rights, given and explained.
