Licensing Program Analyst (LPA) Cynthia Reyes, conducted an announced pre-licensing site Inspection to inspect and measure the requested Infant component for capacity determination. LPA met with Shauna Epting, Director. Due to COVID- 19 precautionary measures were taken during the entire inspection and all individuals present during this inspection wore appropriate personal protective equipment. The licensee is requesting a capacity of 12 Infants (0 to 2 years). Days & Hours for the proposed infant center will be Monday-Friday 6:15 AM-6:30 PM

At 12:38 AM, LPA began the tour of the facility with the Director Shauna, beginning at the front entrance to the entire facility. Director states parents will enter and exit the facility and stop at a table located at the right of the entrance lobby area in front of the double opening doors to the child care center part of the building. The table will be used at this time for Covid-19 for parents to sign the children in/out and temperatures will be taking of the children. Facility was observed to have clean pens available along with an area to place the used pens. (According to CDC guidelines, children under the age of (2) are not required to wear masks.) Staff will then be called by walkie talkie to lead the children to their class rooms. Parents will not enter the class rooms at this time unless they request to enter, where at that time all precautionary measures will be taken for that individual entering. Facility was observed to be adhering to local public health guidelines. LPA observed a designated all day maintenance person on site cleaning the entire facility.

At the front of the double doors to the right is the children isolation room, that consist of a bathroom with toilet and sink and a cot and small chair was also observed. The isolation area will be used by all components, however the facility has 3 other offices that can be used and are located close to a bathroom that children can use if multiple children are ill.

The tour continued through the double doors where the Infant class room is located at the end of the hallway. Each class room door has labels designating what room is used for the infants and what rooms are used for the preschool (F# 198019117) and school age (F# 191501150) components also located on this site.

1:15 PM Licensing staff was guided through a tour of the proposed infant classroom. The classroom was observed to have a separate 4ft. high see through plexiglass wall area for napping. Licensing staff observed the napping area to contain (04) cribs and (06) cots. There is a refrigerator available in the classroom for storing infant formula, bottles, and infant food. A microwave was also observed as well as baby bottle warmers. There is a changing table with cushioning and 3" sides within arm’s length reach of a sink. Play equipment, furniture and materials for infants were inspected to be in good repair and inspection was also for cleanliness, safety and to be age appropriate. There is a fully equipped first aid kit available in the classroom. Individual storage space was observed to be available for each child. A designated area was observed for each child's needs and services plan. LPA observed a bathroom located in the infant class room to have one toilet and one sink.

Telephone service, heating, lighting and ventilation were evaluated. LPA observed there is fire extinguishers, smoke detectors and a carbon monoxide detector on site that meets statutory requirements. No disinfectants, cleaning solutions, and other items that are dangerous to children were observed to be accessible. This program will provide all meals (Breakfast, Lunch & PM snack. As well as formula if needed). this facility is part of a food program. A monthly snack menu is available and posted. LPA advised the licensee to label and date all food, dishes and bottles that are brought from home for proper storage. No transportation is being conducted by the facility. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.



At 1:45 PM, LPA was guided though the outdoor play area. LPA observed that the infant will need to walk though the preschool play yard to get to their own play yard. LPA informed the Director that a waiver will need to be requested for a staggered play schedule between the preschool and infants and a request to walk though the preschool play yard to prevent commingling between programs will be requested. Water bottles and zippy cups with labels with be used for each infant while outdoors. LPA observed are appropriate outdoor toys, as well as adequate shade to be available. All areas around play equipment and slides are cushioned with material to absorb a fall.

2:05 PM Items discussed during this Inspection:
Current Children’s Roster LIC 9040: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This children roster must be updated and available to the licensing agency upon request.
Pediatric First Aid and CPR: At least one person in the facility, along with the Director/Administrator must be trained in Pediatric First Aid and CPR and present in the facility.
Qualifications and staff files: Educational background, training, and/or experience and all required documents by the department for each staff present must be available for review.
Children’s Records: All required licensing documents must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Director advised to visit www.shotsforschool.org for immunization information.
Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or
volunteer at a day care center if he or she has not been immunized against influenza, pertussis, & measles.
Criminal background clearance: LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.
Reporting Requirements Section 101212: Changes at the facility must be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Infant Needs and Services Plan: The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy.


Infant Care: LPA advised the applicant to sleep infants where they can be directly supervised at all time. LPA advised against sleeping infants in a separate room. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts were provided. Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. 2:40 PM LPA advised the applicant how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA also consulted with applicant regarding COVID-19 health and safety guidelines on this date. Applicant was provided forms to post via email. Licensing staff observed COVID- 19 postings posted in different areas of the day care for parents, as well as for children at eye level in their class rooms and in the bathrooms on this date. During this inspection LPA also discussed PIN 20-06 CCP, Social and Physical Distancing Guidance and Healthy Practices for Child Care Facilities in Response to The Global Corona virus (COVID-19) Pandemic Written in Collaboration with The California Department of Education. LPA reviewed with the applicant the Child Care Covid-19 Self-Assessment guide the applicant filled out and sent into the department on 12/15/2020 before this inspection was conducted.

Based on the LPA observation, the following corrections need to be corrected before a license can be granted.


1) Waiver approval for infants to walk though preschool yard with a stagger play schedule, between infants and preschool and a staggered schedule for infants. 2) Final File review

Based on LPA measurements taken on this date, the capacity will be approved for 10 Infants indoors and 07 infants outdoors. A fire clearance was approved on 04/27/2021 and on file for a capacity of 12 Infants.

2:55PM Exit interview was conducted in person with Shauna Epting, Director. A copy of this report and Appeal Rights were given and discussed, signed and printed on this date. Consultation regarding the licensing state laws and regulations, were conducted through out this inspection.
