On January 27, 2021 at 1:40 p.m.,Licensing Program Analysts (LPAs) Roxana Lopez and Cynthia Reyes, conducted an announced Pre-Licensing inspection to measure this facility for capacity determination. LPAs met with Michelle Gallo, Administrator, who provided LPAs with an indoor and outdoor tour of the facility. The applicant is requesting a capacity of 160 school age children. The main entrance to the facility is located in the front of the school.

The proposed school-age component consists of 8 classrooms: 2, 4, 5, 6, 9, 18, 20, and 26 which is the Multipurpose Room. Facility is currently operating on a waiver. All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken. Sign-In and Sign-Out book is located outside of the entrance door of each classroom. During Covid-19, No parents are allowed inside the classrooms, health checks which include temperature checks are conducted in the main office prior to parents signing children in daily. If children become ill during the course of the day, they will be placed in the isolation area; classroom 6. This program will serve school age children ages 5 to 14. (Tk-8th grade) The school age program will operate 7:00 AM to 6:00 PM, Monday to Friday.

During the tour of the class room, Furniture and equipment were inspected for age appropriateness and good repair. School age children do not nap, however a rest period is offered for the younger school- age children. Toys were observed to be clean and safe and age appropriate. Telephone service, heating, lighting and ventilation were evaluated. Children each have their own cubby to store their belongings.

There are 8 restrooms total in the facility 4 are designated for boys and 4 designated for girls. Pre Covid the total # of sinks is 12 and total # of toilets is 8 as well as 8 Urinals. During Covid the total is 8 sinks. 8 urinals and 8 toilets. Sinks not being used are marked off with caution tape. Drinking water is available via water jug and they refill their personal water bottles.

LPAs observed the facility has a carbon monoxide detector and smoke detector in each classroom. There is a serviced fire extinguisher available in each class room. Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible.

Right now during COVID-19 parents are providing all snacks for the children. Once COVID is over school age children will be using the cafeteria which is licensed under the preschool license. The school age is will be using the cafeteria in a staggered license. LPAs advised the Applicant to label and date all food, dishes and bottles that are brought from home for proper storage. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

LPA's were guided through the outdoor space designated for school-age children, The outdoor space measurements were taken, which were more than 200 feet both width and length. The measurements were more then enough square footage required per child. LPAs inspected the school age playground. Outdoor play equipment was inspected for safety, free of sharp, loose pointed parts, good repair and age appropriateness. The surface of the outdoor activity area was observed to be maintained in a safe condition, and free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. Required shade and fencing were inspected. Drinking water was available outdoors via personal water bottles. Children practice social distancing outdoor. Per director the play structures are used and are cleaned after each group of children use.

Applicant states that any medication will be stored in a locked cabinet in each room the that is inaccessible to children in care. First Aid kits are available in each classroom and were observed to contain the required first-aid supplies.

Items discussed during this Inspection:
Licensing does not tolerate the following: Refused Entry to a Facility or Any Part of a Facility is a
violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
Exit interview was conducted in person with applicant Michelle Gallo. A copy of this report (LIC 809) and Appeal Rights (LIC 9058) were discussed, signed and printed and given to applicant on this date. Consultation regarding the licensing state laws and regulations, were conducted through out this inspection.
Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
Current Children’s Roster LIC 9040: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This children roster must be updated and available to the licensing agency upon request.
Pediatric First Aid and CPR: At least one person in the facility, along with the Director/Administrator must be trained in Pediatric First Aid and CPR and present in the facility.
Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
Qualifications and staff files: Educational background, training, and/or experience and all required documents by the department for each staff present must be available for review.
Children’s Records: All required licensing documents must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Director advised to visit www.shotsforschool.org for immunization information.
Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or
volunteer at a day care center if he or she has not been immunized against influenza, pertussis, & measles.

Criminal background clearance: LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.
Reporting Requirements Section 101212: Changes at the facility must be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

LPAs advised the Applicant to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

All precautionary measures regarding Covid-19 were taken by LPAs on this date and discussed with the applicant. LPA's did observe 6ft markers in the classrooms to promote social distancing. LPA's requested for all COVID-19 postings to be placed in all the bathrooms and classrooms at the children's level.

Recommendations: LPA's recommend a fence to be place in front of the perimeter of the school in order to make the classrooms and outdoor space inaccessible to the public, and to prevent a child from walking out onto the heavy traffic street of Loraine Ave.

LPAs advised the Applicant to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Licensee was advised that each component is separate and no parent. staff or children may cross a component to get to another component.

A File Clearance was granted on 10/9/2020 for a capacity of 160. Based on LPA's Measurements the facility will be approved for the requested 160.

Based on the LPA’s observation, the following corrections need to be corrected and final file review before a license can be given. Corrections are due by 09/10/2020:

1) COVID 19 postings in classrooms and bathrooms. Send proof.
