Licensing Program Analyst (LPA) Cynthia Reyes, conducted an announced Pre-Licensing site Inspection to inspect and measure the requested Infant and Toddler Option for capacity determination. LPA met with Joy & Crystal Cyprian, Directors, who also assisted LPA with measuring. Due to COVID- 19 precautionary measures were taken during the entire inspection and all individuals present during this inspection wore appropriate personal protective equipment. The applicant is requesting a capacity of 16 Infants (0-2 years) and 12 Toddlers (18-36 months). Days & Hours for the proposed Infant and Toddler Option will be Monday- Friday 6 AM-6:30 PM. The Center is located in the corner portion of a strip mall called President Plaza. The Center is suite I. The front door faces the parking lot of the strip mall.

At 2:45 PM, LPA began the tour of the facility with the Directors Joy & Crystal, beginning at the front entrance to the facility. Director states parents will enter and exit the lobby from the parking lot. A counter is located inside with a portable hand washing station. During Covid-19 parents will drop off and pick up, as well as sign the children in/out with in the lobby of the facility. Staff will take temperatures of children and parents and hand sanitizer will be available. Parents will not enter the class rooms at this time unless they request to enter, which at that time all precautionary measures will be taken for that individual entering. Facility staff will walk the child inside the class room and bring the child out at pick up time. Parent board is located in the lobby with all the licensing documents and Covid-19 posting. Facility was observed to be adhering to local public health guidelines. (According to CDC guidelines, children under the age of (2) are not required to wear masks.)

The Directors office will be used as an ill/isolation area and has a cot for a child to rest if needed. Ill children will use the staff bathroom located in the hallway next to the office.

3:10 PM, Licensing staff was guided through the proposed Toddler Option class room 1, which is located right next to the lobby entrance door. Cots will be used for napping for all toddlers and are located in a storage shed outside in the back play yard. This room has 1 sink. Their is a bathroom with 2 toilets and 2 sinks in between class room 1 and 2. Room 2 is for the older Infants which was also toured and has a sink within arms reach of a changing table with cushioning material. The bathroom that is in between class room 1 and 2 is separated by a half wall and the applicant will block off part of the wall with a baby gate.

The Infant class room was also toured and there is a changing table with cushioning material within arm’s length reach of a portable sink that is attached to the wall. Staff will dispose of solid waste in the staff rest room located right across from the infant class room or outside in trash bins right away. LPA observed a separate 2 ft. 9 " high see through plexy glass wall area for napping with 6 cribs. Director stated she will remove 2 cribs and add a chair for staff to sit while inside the napping area supervision the napping infants. A bottle warmer was observed in the room, however the facility has a small kitchen located across from the infant room to warm up drinks and food if needed.

Play equipment, furniture and materials for infants/toddlers were inspected to be in good repair as well as for cleanliness, safety and age appropriate. There is a fully equipped first aid kit available in each classroom. Individual storage space was observed to be available for each Infant/Toddler. A designated area was observed for each child's needs and services plan. Facility will provide the linen and has a washer and dryer to clean the linen each week or as often as needed. Telephone service, heating, lighting and ventilation were evaluated. LPA observed there is fire extinguishers, smoke detectors and a carbon monoxide detector on site that meets statutory requirements. No disinfectants, cleaning solutions, and other items that are dangerous to children were observed to be accessible. Facility will provide breakfast, lunch and PM snack. A monthly menu is available and posted. LPA advised the licensee to label and date all food, dishes and bottles that are brought from home for proper storage. No transportation is being conducted by the facility. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

At 4 PM, LPA was guided to the outdoor play area. The Infant and Toddlers will walk out of their class room to the end of the hall out the back door to their shared outside play yard. Waiver has been submitted for a shared staggered play time of the out door play space. Infants and Toddlers will have a water jug and cups for drinking while outdoors. LPA observed age appropriate outdoor toys and play equipment to be clean safe and in good repair, as well as adequate shade available. All areas around play equipment and slides have cushioned material to absorb a fall. Total outside capacity for both components will be for 14 only Infants or Toddlers. LPA advised the Director that the children need to be within the direct care and supervision, including visual observation and supervision of the teacher(s) at all times.

Items discussed during this Inspection:


Current Children’s Roster LIC 9040: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This children roster must be updated and available to the licensing agency upon request.
Criminal background clearance: LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.
Reporting Requirements Section 101212: Changes at the facility must be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Infant Needs and Services Plan: The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. Infant Care: LPA advised the applicant to sleep infants where they can be directly supervised at all time. LPA advised against sleeping infants in a separate room. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts were provided. LPA discussed and provided a copy of PIN 20-24-CCP

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. 5 PM, LPA advised the applicant how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA also consulted with applicant regarding COVID-19 health and safety guidelines on this date. Applicant was provided forms to post via email. Licensing staff observed COVID- 19 postings posted in different areas of the day care for parents, as well as for children at eye level in their class rooms and in the bathrooms on this date. During this inspection LPA also discussed PIN 20-06 CCP, Social and Physical Distancing Guidance and Healthy Practices for Child Care Facilities in Response to The Global Corona virus (COVID-19) Pandemic Written in Collaboration with The California Department of Education. LPA reviewed with the applicant the Child Care Covid-19 Self-Assessment guide the applicant filled out and gave the department on 03/22/2021 at this inspection as it was conducted.

Based on the LPA observation, the following corrections need to be corrected before a license can be granted.


(1) Waiver approval for a staggered and shared outside play space, due to measurements only allow for a total of 14 children outside at one time (2) Photos of the Infant napping with a 4 ft. high patrician. (3) Baby gate to separate the bathroom for toddlers and infants. (4) Final File review.

Based on LPA measurements taken on this date, and once all correction are made the capacity will be approved for capacity of 14 Infants (0-2 years) and 10 Toddlers (18-36 months). A fire clearance was approved on 12/25/2020 and on file for a total capacity of 28 Infant/Toddlers. Approved Total Capacity will be for 24

5:50 PM, Exit interview was conducted in person with Joy & Crystal Cyprian, Directors. A copy of this report and Appeal Rights were given and discussed, signed and printed on this date. Consultation regarding the licensing state laws and regulations, were conducted through out this inspection.
