25 | PRELICENSING INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an announced pre-licensing inspection to the above facility on 03/11/2022. LPA arrived at the facility at 8:40 AM and met with Taisa Haykyan, Applicant who guided analyst on a tour of the facility. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per applicant operation hours will be Monday to Friday, 7:30 a.m. to 8:00 p.m.. Applicant states she will care for children 1-12 years old.
Applicant was previously licensed at 823 E. Windsor Rd., Glendale, CA 91205 with facility number 198018398. Fire clearance for Large Family Child Care was granted on 03/02/2022.
All areas identified on the facility sketch were inspected. This is a one story home located on the first level. The home consists of 3 bedrooms, 3 restrooms, living room, kitchen, permitted converted porch to the room (main day care area), front yard, detached garage, and backyard (fenced).
Areas that are accessible to children are as follows: Living room, kitchen, bedroom #3 adjacent to the converted porch, bathroom in the bedroom #3, converted porch (main daycare area), and front part of back yard (fenced), which has been separated by four feet high vinyl gates from off-limit areas in the backyard.
Areas off limits based on facility sketch submitted to children and parents include: Two bedrooms, two bathrooms, back of backyard, garage and front yard..

Per applicant parents will enter the facility either from the main entrance which leads to the living room or from the side gate which leads to the backyard and main daycare room. The applicant does understand that licensing staff may have access to off-limit areas during inspection visit if necessary.
