On March 9, 2021 at 1:00 PM, Licensing Program Analyst (LPA) Monique Ayala conducted a Prelicensing tele-inspection via Zoom Meeting and met with Licensee/Applicant, Diana Sanchez. The inspection was conducted via Zoom Meeting due to the Coronavirus Disease 2019 (COVID-19) pandemic. The facility is currently licensed for a capacity of 20 preschool children. This program operates Monday through Friday from 6:30 AM to 6:00 PM.

The Licensee submitted an application to increase the capacity to 32 children, ages 2 to entry into kindergarten. The facility consists of 2 buildings; Building A and Building B. The facility has an outdoor play yard located between the two buildings. Building A consists of an office, 3 classrooms and 2 restrooms. The 2 restrooms in Building A each have 1 sink and 1 toilet for children use. Building B consists of a main entry way, teacher's lounge, 2 classrooms and 1 restroom. The restroom in Building B has 1 toilet and 1 sink for children use.

LPA was guided on a virtual tour of the facility. All indoor and outdoor areas identified on the facility sketch were inspected and measurements were taken.

First Aid supplies were inventoried. A first aid kit with a thermometer is kept in the front office. Carbon monoxide detectors, smoke detectors and serviced fire extinguishers are present and in operable condition.

The program provides all food items that are prepared at the facility. A weekly menu is available and posted. The facility will administer prescription and non-prescription medications. Medications will be stored in the front office in a locked box. The main entrances inside the buildings will be used as an ill/isolation area, which has a cot available if needed. The facility does not have a staff restroom located in the teacher's lounge; therefore, there is not a separate restroom available for use by ill children. Per Licensee/Applicant, the staff restroom will be under construction this weekend.

Sign-In and Sign-Out procedures will be conducted at the entrance of the facility. Furniture and equipment was inspected for age appropriateness and good repair. Children have their own cubby to store their belongings. Napping equipment (cots) was observed. Per Licensee/Applicant, linen will be taken home each week to be washed. Age appropriate sinks and toilets were inspected for availability and good repair. Drinking water is readily available in each classroom. LPA observed storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Cleaning compounds and any hazardous items that are dangerous to children are stored in areas that are off-limits to children. Per Licensee/Applicant, the facility does not have any poisons.

At the time of the inspection, LPA observed a refrigerator and stove located inside Classroom 3 of Building A. Per Licensee/Applicant, Classroom 3 is not currently in use due to low enrollment and confirmed that the kitchen is located inside Classroom 3. LPA also observed the classrooms with two entry and exit ways without doors. Each classroom was open to one another and did not have barriers to separate the classrooms.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed material under play equipment that could absorb a fall. There is adequate shade in the play yard. Water pitchers and disposable cups are available for the children in care. There are no bodies of water on the premises. LPA advised the Licensee/Applicant that children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. I. Licensing does not tolerate the following: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
II. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
III. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
IV. Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
V. Designated Staff: The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
VI. Qualifications: Educational background, training, and/or experience for each staff present must be available for review. Per Licensee/Applicant, Teacher’s will have a copy of their teacher permit available.
VII. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
VIII. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.
IX. LIC 311A - Records to be Maintained at the Facility: Child Care Centers was provided to the applicant.
X. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.
XI. Provider Information Notice (PIN) 20-01-CCP: Required Lead Testing for Drinking Water in Licensed Child Care Centers and the Provision of Lead Exposure Information to Parents and Guardians by All Licensed Child Care Facilities.

LPA advised the Licensee/Applicant that forms, regulations, quarterly updates and Provider Information Notices (PINs) can be accessed on the Department website at www.ccld.ca.gov.

On March 9, 2021, the Licensee/Applicant submitted the completed Child Care Program COVID-19 Self-Assessment. During the inspection, LPA reviewed the implementations noted on the guide and discussed plans to ensure that the program will meet COVID-19 guidance for a child care setting.

LPA observed that the facility has hand sanitizer readily available. Licensee/Applicant was advised that children may use hand sanitizer but only under adult supervision and hand sanitizer must be kept out of children's reach. Upon arrival, the program conducts a health screening of the children by taking the child's temperature and asking the child's parent or guardian questions regarding any confirmed cases of COVID-19, COVID-19 exposure or COVID-19 symptoms.

The program disinfects and sanitizes high touched areas after each use, follows physical distancing guidelines, requires face coverings for adults and children over the age of 2, and adheres to COVID-19 specific exclusion criteria. The program plans to introduce fresh outdoor air as much as possible, offer opportunities for individual play, and adjust schedules to reduce the number of children in the same area. The program teaches the children to avoid contact with one's eyes, nose and mouth, and use tissue to wipe their nose and to cough or sneeze inside their elbow. The program also models and practices hand washing before and after eating, after coughing or sneezing, after playing outside, and after using the restroom.

The COVID-19 guidance for child care settings is discussed with families and staff. LPA observed that the facility had gloves readily available, which are used by staff. The Licensee/Applicant was advised to report any possible exposures and/or positive cases of COVID-19 to the Regional Office. The Licensee/Applicant was also advised that staff and families must be notified immediately of any possible cases of COVID-19 while maintaining confidentiality.

The fire clearance was granted for 16 preschool children and 16 toddlers on January 15, 2021. The Licensee/Applicant is seeking to provide care for 32 preschool children. Based on measurements taken today, the maximum capacity allowed will be 31 preschool children.

The following corrections must be made for further processing:
  1. A new fire clearance must be submitted and approved to reflect the capacity of 31 preschool children.
  2. An updated Facility Sketch that shows the true indoor floor plans of the facility.
  3. The facility must have a separate toilet and sink available for staff and ill children.
  4. Barriers need to be installed between each classroom to separate the classrooms from one another.
  5. The kitchen space can not be located inside a classroom; therefore, the stove and food preparation area must be moved to another area that is not used by children.
Per Licensee/Applicant, corrections will be completed by Thursday, March 25th. LPA will schedule a follow-up inspection to ensure that corrections are complete.

An exit interview was conducted and a copy of this report will be issued to the Licensee/Applicant via e-mail. Per Licensee/Applicant, the signed copy of the report will be submitted to the Monterey Park South West Child Care Regional Office by Thursday, March 11th.
