Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced case management inspection- "change of capacity" to the above facility. Licensing staff met with Latania Young Howard, Licensee who guided analyst on a tour of the facility. Also present during this inspection was Licensee’s Assistants, Maria Calderon and Rose Calderon. Tour began at 11:40am. The licensee states that she currently has 8 children enrolled. During this inspection there were 3 children and 3 infants present. A children’s roster is available and is current. Per Licensee, hours of operation are 6am to 6pm, Monday to Friday.

This is a one story home that consists of 4 bedrooms, 2 restrooms, dining room, living room, den, front room kitchen, garage, backyard (backyard is split into two sections), and front yard.

Per Licensee, children use the restroom in the hallway (near the dining room), the front room (living room), the den (nap room), 1 bedroom ( the bedroom near dinning room, this bedroom is used for an isolation area only), and backyard. Areas that are used by children were inspected for safety, comfort, cleanliness, telephone service (cell phone & land line), ventilation and heating (central). Detergents, cleaning compounds, medicines, sharp objects and hazardous items that can pose a danger to children are inaccessible in areas designated for children and are located in high cabinets of kitchen.
Areas off limits to children and parents are: Three bedrooms, one bathroom, kitchen, garage, half of the backyard, and front yard. There is a safely knobs and safety gates on off- limit areas. The applicant understands that licensing staff may have access to off-limit areas during inspection visit if necessary.

The children will use half the back yard for outdoor play, which was observed to be fenced. Licensee which states that children will be physically and visually supervised at all times.

Fireplaces and open face heaters are inaccessible. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 04/26/2021, as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. Smoke and Carbon detector were tested and were operable. All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 12/13/21. The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR which expire 04/28/2023

Infant Care: Currently licensee cares for 4 infants enrolled which 2 are over the 12 months. LPA observed 4 play yards visible in nap room and 1 in the living room. Napping equipment does not block entrances or exits. Infant mattresses were observed to be firm with tightly fitted sheets. LPA did not observe loose object, bumpers, objects hanging, or objects attached to the play yards. There are currently no infants with pacifiers. Each infant has their own play yard and bedding. LPA informed licensee of the new Safe sleep regulations, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0-24 months, and provided PIN 20-24-CCP. Licensee states the following a sleep supervision plan for infants: they sleep in the nap room.

LPA's observed that the toys and other materials for children to play with. LPA's did not observe any objects that could be hazardous to children in care. There are no pools or spas, or other bodies of water. Licensee stated there are no weapons or firearms in the facility.


Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunizations Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 9227- Infant sleep form (0-12 months, and documentation of 15-minute Infant Sleep Check (0-24 months).

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement of Child and Mandated Reporter Training Certificate.

During inspection all children were observed to be treated with dignity and respect, they were observed to be receiving safe, healthful and comfortable accommodations, furnishings and equipment, and free from corporal and/or unusual punishment.

LPA observed that licensee is implementing COVID-19 precautions and procedures.

· Inspection Authority: All adults living and working in the home shall be made of aware of the Department’s right to inspection the home, which includes, but is not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.
· The facility license number must be on all advertisements, publications or announcements with the intent to attract clients.
· Isolation for Ill children: When a child is ill he/she shall be separated from other children (reference 102417(e) Operation of a Family Child Care Home). Per applicant, isolation area will be in the bedroom near the dining room.
· Liability Insurance was discussed; LPA advised applicant to review Title 22 Regulation 102417(m)(1) for additional information. Licensee does not have liability insurance at the moment but is considering it. Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The licensee and all adults working with children have proof of immunizations. Mandated Reporter Training: H&S 1596.8662: Beginning January 1, 2018, all licensed providers, applicants, directors and employees to complete training as specified on mandated reporter duties. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. LPA advised the applicant how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA reviewed and issued the Forms/Records to Keep in Your Family Child Care Home (LIC 311D) and provided the following forms:
CHILDREN FORMS/RECORDS - Children’s files must contain the following documents/information:
Identification and Emergency Information (LIC 700), Consent for Emergency Medical Treatment (LIC 627), Notification of Parent’s Rights (LIC 995A), Caregiver Background Check Process (LIC 995E), Family Child Care Consumer Awareness Information (LIC 9212), Consent/Verification for Nebulizer Care (LIC 9166), California School Immunization Record, Parent Notification for Additional Children in Care (LIC 9150), Affidavit Regarding Liability Insurance (LIC 282), Acknowledgment of Receipt of Licensing Reports (LIC 9224).

A Notice of Site Visit (LIC 9213): Must be posted for 30 days after each site inspection by a Licensing Representative. Any Licensing Report documenting a Type “A” deficiency must be posted for 30 days during the hours that children are in care. Any Licensing Report or other document verifying compliance or non-compliance with the Department’s order to correct a Type “A” deficiency must be posted for 30 days during the hours that children are in care.
OTHER INFORMATION AND FORMS PROVIDED:
· Handouts provided for Never Shake a Baby, Sudden Infant Death Syndrome (SIDS) and Safe Sleeping practices
· Capacity Handout for a Large Family Child Care Home was provided.

A large family child care licensee will be granted once the department does a final review of the application. .
