Licensing Program Analyst (LPA) Denise Gibbs conducted an announced pre-licensing inspection on 7/27/21 at 1:00PM. LPA met with Dr. Linda Johnson, Applicant who guided analyst on a tour of the facility.

This is a proposed Infant Program with Toddler option which consists of 2 classrooms and a nap room. The Infant program will serve children ages, zero to 24 months old and the Toddler option will serve children 18 months to 36 months old with guardian consent. This program will operate Monday - Friday 6:30 AM - 6:00PM. This facility may also contain a Preschool component, License #198400296.

All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken.

Required postings will be visible in the entrance of each classroom. Due to COVID-19 precautions documents will also be posted in parent drop off area in the main entrance. LPA observed all required documents printed. Sign-In and Sign-Out will be conducted by use of the Bright Wheel application. If application is down, manual sign in/out will be used. Health checks will be conducted before children enter the facility. Temperatures will be taken daily for COVID-19 precautions.

LPA observed that the infant and Toddler classrooms are physically separate from each other and from the Preschool areas. A waiver will be requested for shared space in the infant and toddlers yard. Each component has a separate entrance for parents.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated and are operable. Children will have their own cubby to store their belongings. Linens will be changed daily at the facility or more frequently as needed. Infant napping equipment (cribs and cots) were observed. LPA observed 10 cribs visible and free of hazards in a separate nap room. -----------------PAGE 1

Per Applicant, there will always be a qualified teacher and assistant (when needed) present in the nap room while infants are napping. Toddlers will nap on cots in their classroom.

LPA observed two restroom, one in the infant room and one in the toddler room. Restrooms were observed to be clean with two working toilets and two working sinks. Infant changing tables have a padded surface no less than one inch thick and are covered with washable vinyl and have raised sides at least three inches high. The changing table was observed to be within arm’s reach of a sink. LPA observed that facility does have a carbon monoxide detector, smoke detector, and a serviced fire extinguisher in every classroom. Drinking water will be provided using water jugs and cups both indoor and outdoors.


Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible and stored in locked storage room at the facility.

Per applicant, the isolation area is located in the Director's office right next the Toddler room. There will be a cot available for children to rest if needed. As of now applicant states that medication will not be administered at the facility due to COVID-19 precautions, this may change in the future. First aid kits are available in each classroom and in the main entrance to the facility.

This program will provide Breakfast, Lunch and PM snack for the Toddler Program and Older infants who are ready for table food. Meals and snacks will be delivered daily from a partnered restaurant in individual servings and containers. Extra snacks will be available on hand at the facility. A weekly Lunch/Snack menu is available and will be posted. LPA advised Applicant that all food, dishes and bottles that are brought from home must be labeled with the child’s name and date for proper storage. Infants have age appropriate feeding chairs/tables. LPA advised Applicant to ensure that infants are being visually supervised during feeding times. Infant puree food and cereal will be provided by facility. Bottles and food from parents will be stored in a refrigerator in the classroom.

Per applicant kitchen and food prep areas will rarely be used do to catering service. All kitchen/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste have tight-fitting covers that are kept on and are in good repair. ------------------PAGE 2

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor play space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. LPA advised applicant that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA advised applicant to use barriers in two areas on the play yard, one leading to the parking lot and one leading to a blind spot behind the play yard. Per applicant material will be added to fencing around the gate of the play. LPA observed wide gaps in the gate, which could pose a hazard. There are no bodies of water or firearms on the premises.


Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. Items discussed during this visit:


LPA provided the following documents about SIDS: A Child Care Provider’s Guide to Safe Sleep from the American Academy of Pediatrics, Helping You to Reduce the Risk of SIDS, Never Shake a Baby, Lead Pamphlet and Safe Sleeping practices. Prohibited items: baby walkers, baby bouncers, inclined sleepers or baby saucers at the facility. LPA discussed Individual Feeding Plans and Infant bottle preparation with Applicant.

Additional Items discussed during inspection: COVID-19 self-assessment and precautionary procedures. Applicant provided LPA with completed self-assessment on 7/27/2021. The following was discussed:

Essential Protective Equipment and Supplies- Applicant has a contact-less thermometer and a supply of disposable face coverings, gloves, hand sanitizer and disinfectant cleaning supplies at the facility. Children and adults will be required to wear facial coverings throughout the day.


Cleaning- Per applicant, there is a dedicated staff that will work throughout the day to disinfect high touch areas and cleaning throughout the facility. Teacher's assistants will also help with this task.
Hygiene- Per applicant hand washing will be a part of the curriculum that they will be working on throughout the day. Hand sanitizer will be available when they cannot get to the sink right away. Per applicant personal items ---------------------PAGE 3

will by limited, hygiene items will be provided by facility. Lunched will be provided in individual servings. The isolation area for a sick child waiting to be picked up will be in the overflow room near the exit.
Arrival Procedures- Per applicant parents will drop off/pick up in the covered patio area. New parents will be able to talk to teachers by appointment. Staff will do daily temperature checks and health screening at the front door, as parents will not be allowed to enter the facility. COVID-19 posters and resources are posted in the front entrance accessible to parents. Parents will be made aware of the COVID-19 policies and exclusion procedures initially through the enrollment process, the parent handbook and a group orientation before children begin.
Distancing/Meals/Sleep/Outdoors- Per applicant they will be doing a lot of outdoor activities. They will practice distancing and implement activities into the curriculum so that distancing will be fun. They will have staggered play throughout the day where small groups will be inside and small groups will be outside. For meals they will also use the outdoor as much as possible. When indoors meal times are needed staff will reinforce social distancing.

The applicant is seeking to provide care for 10 infants and 12 Toddlers; however, based on measurements taken today, applicant can provide care for seven infants and 12 Toddlers. A fire clearance has been granted as of 7/9/21.

Based on the LPA’s observation, the following corrections need to be corrected prior to obtaining a Infant childcare license.

  • Two child safety barrier in the outdoor yard
  • Material added along outdoor gate.
  • Shared Yard Waiver

A Child Care license may be granted upon receipt of proof of corrections and Licensing Program Manager (LPM) Approval. Once licensed, the applicant is required to comply with the terms and limitations stated on the license.
