This was Pre licensing inspection conducted on 11/13/2020 at 2:30 PM by Denise Gibbs, Licensing Program Analyst (LPA). Due to COVID-19 and precautionary measures, this Pre licensing Inspection was conducted with Latanya Collier, applicant via a tele-inspection by use of Facetime.

This is a proposed Infant Program which consists of one classroom. This facility houses a preschool program license #198019480. This program will serve children ages, zero to 24 months old. Operating hours will be Monday - Friday 7:00 AM - 6:00 PM.

All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken.

LPA observed required licensing postings. Sign-In and Sign-Out will be conducted by use of the Bright Wheel application. If application is down, manual sign in/out will be used. Health checks will be conducted as children enter their classrooms daily. Temperatures will be taken daily for COVID-19 precautions.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated and are operable. Children have their own cubby to store their belongings. Linens will be changed daily at the facility or more frequently as needed. Linens with be washed by facility staff. Infant napping equipment (cribs and cots) were observed. LPA observed eight cribs visible and free of hazards. The brand of cribs is Child Craft. Per Applicant, there will always be a qualified teacher present in the nap area while infants are napping. Older infants will sleep on cots.

Age appropriate sinks and toilets were inspected for availability and good repair. Infant changing table has raised sides at least three inches high and a vinyl pad for sanitizing. The changing tables -----------------PAGE 1

were observed to be within arm’s reach of a sink. LPA observed that facility has an operable carbon monoxide detector, smoke detector, and a serviced fire extinguisher. There is drinking water readily available in the classroom via water fountain.

Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible and stored in the kitchen, under the sink and in the maintenance room. Applicant states that poisons are stored at the facility.

Per applicant, the isolation area is located in the office; there is a cot available for children to rest if needed. Applicant states that medication will be stored in a locked box in classroom, inaccessible to children. First aid kit is available in the classroom and in the office.

This program will provide an breakfast, Lunch and a PM snack. Parents are required to bring infants meals until infants can eat whole foods. The facility will be providing food for the older infants. A monthly snack menu is available and posted. LPA informed Applicant that all food, dishes and bottles that are brought from home must be labeled with the child’s name and date for proper storage. Infants have age appropriate feeding chairs/tables. LPA advised Applicant to ensure that infants are being visually supervised during feeding times.

All kitchen/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste have tight-fitting covers that are kept on and are in good repair.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor play space is maintained in a safe condition and is free of hazards. At 3:00 PM LPA observed that the infant play area is not gates and it does not have a shaded area. Per applicant contractor has measured and a gate will be put up within the next week, along with a shade. LPA informed applicant that the children need to be within the direct care and visual supervision of the teacher(s) at all times. There are no bodies of water or firearms on the premises.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. -------------------PAGE 2

LPA provided the following documents about SIDS: A Child Care Provider’s Guide to Safe Sleep from the American Academy of Pediatrics, Helping You to Reduce the Risk of SIDS, Never Shake a Baby, Lead Pamphlet and Safe Sleeping practices. Prohibited items include: baby walkers, baby bouncers, inclined sleepers or baby saucers. LPA discussed Individual Feeding Plans and Infant bottle preparation with Applicant.

LPA Provided COVID-19 Technical Assistance by emailing applicant COVID-19 resources and self-assessment prior to visit. LPA discussed self-assessment and answered question regarding COVID-19 procedures. LPA observed advised supplies and screening materials.

Applicant is seeking to provide care for 16 Infants; Based on measurements, the facility has space for: 16 Infants Indoors and 11 Infants outdoor. Applicant has requested an Outdoor waiver for staggered play. A fire clearance has been granted as of 11/10/2020.

Based on the LPA observation, the following corrections need to be made prior to obtaining an Infant license.

  • Four foot minimum gate needed to separate infant yard from preschool yard
  • Shade needed for outdoor yard
  • Pending missing documents
  • Outdoor waiver requests

A Child Care license may be granted upon receipt of proof of corrections and Licensing Program Manager (LPM) Approval. Once licensed, the applicant is required to comply with the terms and limitations stated on the license.
