This was Pre licensing inspection conducted on 3/2/2021 at 2:00 PM by Denise Gibbs, Licensing Program Analyst (LPA). Due to COVID-19 and precautionary measures, this Pre licensing Inspection was conducted with Margaret Pettis, applicant via a tele-inspection by use of Facetime.

This is a proposed Preschool Program which consists of Three classroom. This program will serve children ages, Two to six Years Old. Operating hours will be Monday - Friday 5:00 AM - 11:59 PM.

All areas identified on the facility sketch were toured both indoors and outdoors. Measurements of the indoor and outdoor space were not taken due to confusion over televisit versus physical visit. Measurements will be taken during follow -up inspection.

Required postings will be visible in the main entrance area. Per applicant, the board will be put up within the week. LPA observed all required documents printed. Sign-In and Sign-Out will be conducted by use of the Bright Wheel application. If application is down, manual sign in/out will be used. Health checks will be conducted before children enter the facility before they get to the classroom. Temperatures will be taken daily for COVID-19 precautions.

LPA observed three classrooms, Two's Room, Three-Four's Room and Five-Six Year old Room. Furniture and equipment was inspected for age appropriateness and good repair. Each classroom was observed to have age appropriate materials. Telephone service, heating, lighting and ventilation were evaluated and are operable. Children have their own cubby to store their belongings. Linens will be washed weekly (every Friday) at the facility or more frequently as needed. LPA observed different size cots for each age group.

LPA observed two restroom, one for the older children in the Five-Six year old classroom and one for the younger children between the Twos and Three-Four year old room. Restrooms were observed to be clean with four ---------------PAGE 1

working toilets and three sinks. LPA observed that one restroom stall in the younger restroom had a broken toilet paper holder and there was no paper towel dispenser. Per applicant they will have that fixed as soon as possible.

LPA observed that facility has an operable carbon monoxide detector, smoke detectors, and a serviced fire extinguisher. Drinking water will be provided using a pitcher and cups both indoor and outdoors.

Disinfectants, cleaning solutions, and other items that are dangerous to children will be in an inaccessible in the kitchen, locked under the sink. Applicant states that poisons are not stored at the facility. All storage containers for solid waste have tight-fitting covers that are kept on and are in good repair.



Per applicant, the isolation area is located in the office; there will be a cot available for children to rest if needed and a restroom close by. Applicant states that medication will only be administered with a prescription from a doctor and it will be stored in the kitchen in a labeled box. First aid kits are available at the end of each hallway.

This program will provide AM snack, PM snack and dinner. Lunch will be provided by the child's family. A monthly snack/dinner menu is available and will be posted. LPA informed Applicant that all bags and/or dishes that are brought from home must be labeled with the child’s name and date for proper storage. All kitchen areas/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects and rodents. LPA observed that the kitchen did not have a refrigerator or stove. Per applicant both items will be installed soon.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. LPA's advised applicant that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA observed awning outside of the facility which provides a shaded area. Per applicant they will be adding more shade soon. There are no bodies of water or firearms on the premises.

Incidental Medical Services (IMS): Incidental Medical Services was discussed – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. Additional Items discussed during inspection: COVID-19 self-assessment and precautionary procedures. Applicant provided LPA with completed self-assessment on 3/2/2021. The following was discussed:

Essential Protective Equipment and Supplies- Applicant has a contact-less thermometer and a supply of disposable face coverings, gloves, hand sanitizer and disinfectant cleaning supplies at the facility. Children and adults will be required to wear facial coverings throughout the day.


Cleaning- Per applicant, high touched areas, and bathroom will be disinfected every hour. Toys will be placed in a dirty toy bin and sanitized as needed. all toys will be sanitized daily.
Hygiene- Per applicant children will be encouraged to was their hands for 20 seconds throughout the day as needed. Applicant has the advised children's hand washing and germs posting at their level. Children are required to wash their hands at drop off before entering the classroom. Per applicant personal items are not allowed a the facility at this time. Lunched will be placed in individual cubbies. The isolation area for a sick child waiting to be picked up will be in the Director's off or outside if staff and weather permits..
Arrival Procedures- Per applicant parents will drop off/pick up at the door. Staff will do daily temperature checks and health screening at the front door, as parents will not be allowed to enter the facility. COVID-19 posters and resources are posted in the front entrance accessible to parents. Parents will be made aware of the COVID-19 policies and exclusion procedures though Bright wheel messaging system.
Group Size- Applicant understands that a cohort for this age group is limited to 12 child, one teacher and a floating teacher. Per applicant, the group size will not be exceeded.
Distancing/Meals/Sleep/Outdoors- LPA observed partitions for the children to use while they are playing/eating at the tables. Per applicant they will also use Velcro squares for spacing while sitting on the carpet. Children will use the outdoor area at staggered times. Cots have space for distance and children will sleep head to toe.

Applicant is seeking to provide care for 30 Preschool; A fire clearance has been granted as of 1/4/2021.

Based on the LPA observation, the following corrections need to be made prior to obtaining an Infant license.

A Child Care license may be granted upon receipt of proof of corrections and Licensing Program Manager (LPM) Approval. Once licensed, the applicant is required to comply with the terms and limitations stated on the license.
