Licensing Program Analyst (LPA) Denise Gibbs conducted an announced pre-licensing inspection on 1/13/22 at 9:45 AM. LPA met with Jamey Dogmom, Applicant who guided analyst on a tour of the facility.

This is a proposed Preschool Program which consists of five classroom. This program will serve children ages, Two to six years old. Operating hours will be Monday - Friday 8:00AM - 6:00PM.

All areas identified on the facility sketch were toured both indoors and outdoors. Measurements of the indoor and outdoor space were taken as needed.

This is a two-story facility with detached building. All children's activities will be conducted on the first floor. The second floor will be used for staff only.

LPA observed that required postings need to be put up in facility, LPA provided all postings. Due to COVID-19 precautions documents will also be posted in parent drop off area in the main entrance. Sign-In and Sign-Out will be done manually on the front porch as parents will not enter the facility due to COVID-19 precautions. Health checks will be conducted before children enter the facility. Temperatures will be taken daily.

LPA observed five classrooms, four classrooms in the main building and one detached classroom (back house). Furniture and equipment was inspected for age appropriateness and good repair. Each classroom was observed to have age appropriate materials. All electrical outlets are safety outlets that do not require outlet covers. Telephone service, heating, lighting and ventilation were evaluated and are operable. Children will have their own cubby to store their belongings. Children will bring their own bedding for nap and they will be taken home weekly for families to wash.

LPA observed three restrooms, two in the main building and one in the back classroom. Restrooms were observed to be sanitary with three working toilets and three working sinks. LPA observed cleaning supplies in children's bathroom in the back house that will have to be removed.

LPA observed that facility has an operable carbon monoxide detector, smoke detectors, and serviced fire extinguishers. Drinking water will be provided using filtered water. Children will bring there own water bottles that can be refilled throughout the day. Extra cups will be available if needed.

Disinfectants, cleaning solutions, and other items that are dangerous to children will be inaccessible in a locked storage cabinet and gated storage room. All storage containers for solid waste have tight-fitting covers that are kept on and are in good repair.



Per applicant, the isolation area is located in the back house where the child can be away from other children with an isolated restroom. First aid kits will be available in main building, the back house and outside.

This program will provide AM and PM snack. Children will bring their own lunches. A monthly Lunch/Snack menu is available and will be posted. LPA informed Applicant that all bags and/or dishes that are brought from home must be labeled with the child’s name and date for proper storage. Snacks with be stored in refrigerator and on the snack cart. All kitchen areas/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. Facility will not administer short term medication. Facility will only administer long term or life saving medication with prescription and parents instruction.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. LPA advised applicant that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA observed covered patio area outside which provides a shaded area. LPA did observe lose brick, meatal pipes, pieces of wood and pieces of concrete that will have to be removed. There are no bodies of water or firearms on the premises. LPA observed a shaded area, grass areas and a cement area for children to play. LPA advised that applicant to add a barrier to block the path behind the back house to make it inaccessible to children. Per applicant they will add barriers for safety.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department.

An IMS Plan was provided with the application and will be reviewed.

Additional Items discussed during inspection: COVID-19 self-assessment and precautionary procedures. Applicant provided LPA with completed self-assessment on 1/13/22. The following was discussed:

Essential Protective Equipment and Supplies- Applicant will have a contact-less thermometer and a supply of disposable face coverings, gloves, hand sanitizer and disinfectant cleaning supplies at the facility. Children and adults will be required to wear facial coverings throughout the day.


Cleaning- Per applicant, cleaning and sanitizing will be done daily, a dirty toy bin will be utilized. High touched areas will be cleaned as often as possible.
Hygiene- Per applicant hand washing will be a part of the curriculum that they will be working on throughout the day. Children will be encouraged to wash their hands for at least 20 seconds using songs to help them. The isolation area for a sick child waiting to be picked up will be in the back house where there is a restroom.
Arrival Procedures- Per applicant parents will drop off/pick up in the covered porch area. New parents will be able tour the school before and after the program when children are not present. Staff will do daily temperature checks and health screening at the front door, as parents will not be allowed to enter the facility. COVID-19 posters and resources will be posted in the front entrance accessible to parents. Parents will be made aware of the COVID-19 policies and exclusion procedures initially through the parent handbook. Facility will have a bulletin board outside on the porch with posters and guidelines.
Distancing/Meals/Sleep/Outdoors- Per applicant they will be doing a lot of outdoor activities. Meals will be outdoors as long as weather permits. The eating area is covered. There will be visual cues in the play areas to show distance. They will talk about personal space and germs as part of the curriculum. Children can be spread out for nap at least three feet.

Applicant is seeking to provide care for 45 Preschool; Based on measurements and fire clearance, the facility has room to care for 32 Preschool children. A fire clearance has been granted as of 6/22/2021.

Based on the LPA observation, the following corrections and a follow-up visit is needed prior to obtaining Preschool license.

  • Barrier making walkway behind back house inaccessible
  • Removal of lose brick, wood, metal pipes and any other hazardous items in the front play yard.
  • Removal of cleaning products from the children's restroom in the back house.
  • Removal of all storage items from the back house
  • Add required postings


A Child Care license may be granted upon receipt of proof of corrections and Licensing Program Manager (LPM) Approval. Once licensed, the applicant is required to comply with the terms and limitations stated on the license.
