This was a pre-licensing inspection conducted on 02/03/22 at 11:45 AM by Raul Navarro, Licensing Program Analyst (LPA). Inspection was conducted with Nakia Hammond, Applicant.

Individuals residing in the home have been discussed and noted. The applicant is requesting a Large Family Child Care Home license. Per Applicant, operation hours will be Monday - Friday, 6:00AM-6:00PM. Applicant states that they will care for children Infants to school-age.

During this inspection the applicant took LPA on a tour of the home and the following was noted:


All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a one-story home that consists of three bedrooms and one bathroom. Areas that will be used by the children include the living room, den, bathroom, and backyard. Outdoor play area is located in the back yard. LPA observed that the yard is fenced.

Off limit areas include: three bedrooms, dining room, front yard, and kitchen. Bedrooms are made inaccessible via child safety knobs. The kitchen drawers all have locks as an additional precaution. Steps are made inaccessible using a child safety gate.The applicant understands that licensing staff may have access to off-limit areas during inspection visit if necessary.



Areas that will be used by children were inspected for safety, comfort, cleanliness. LPA observed operable telephone service via cell phone and land line. Ventilation and heating was also observed. Detergents and cleaning compounds are made inaccessible in the locked shed. Per Applicant, there are no medications in the home. The Applicant was advised that any poisons must be locked, not just inaccessible.

There are age appropriate toys and material available for children. Older children will nap on cots in the Main Care area. The bathroom children will use was observed to be free of hazards. LPA observed a fire place in the living room that is screened.

Applicant states that they will care for infants zero to 24 months old. LPA advised Applicant that when a infant is enrolled they will need to have a play yard or crib for the infant. LPA advised napping equipment can not block entrances or exits. Infant mattress shall be firm with tightly fitted sheets. LPA advised that cribs or play yards should not have loose object, bumpers, objects hanging, or objects attached to the cribs or play yards. Bedding will be changed daily and washed as needed.

LPA informed applicant of the new Safe sleep regulations, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0-24 months, and provided PIN 20-24-CCP. Applicant states the following a sleep supervision plan for infants: Staff will always be supervising sleeping infant in main care area.

Per Applicant there are no pets on the premises and there are no firearms or weapons stored in the home. There no bodies of water in the home.



LPA observed that the Applicant has a 2A10 BC fire extinguisher in the home, serviced on 11/21. Smoke/carbon monoxide detector was tested and is in operable condition.

Per Applicant children will bring their own food. LPA informed that any food brought from the children’s homes shall be labeled with child’s name and properly stored or refrigerated.

The Applicant has completed the required Health and Safety Training with one hour of Nutrition and one hour Lead training included. Applicant has completed the Mandated Reporter Training and Pediatric First Aid and CPR which will expire in June 2023. Applicant has proof of immunization against influenza, pertussis, and measles on file. There are first aid supplies available.

Per Applicant parents will not be allowed inside the home. LPA observed health assessment items at the front door. LPA observed sanitizer, thermometer, masks and gloves. Per applicant they will ask screening questions and take children's temperatures daily.

LPA observed posters for parents and children informing about COVID and hand washing posted in the facility. Children will have their own cubby for their personal belongings. Per Applicant children and staff will wear face coverings during day care hours. Children be will encouraged to wash hands for 20 seconds multiple times during the day. LPA observed soap and paper towels in the bathroom. Applicant has posters that show hand washing steps for children. Applicant has a supply of cleaning and sanitizing products. Per applicant high touched areas will be cleaned throughout the day as needed. LPA observed large tables space for activities and meals for social distancing. LPA observed space in the main care area to socially distance children during nap time. If a child becomes ill, applicant will utilize the living room as an isolations area away from other children. LPA observed an outdoor area where child have space to play.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A Large Family Child Care license will be granted upon further review. Once licensed, the Applicant is required to comply with the terms and limitations stated on the license.
