On 01/25/2022, Licensing Program Analyst (LPA) Juvenal Moctezuma conducted an announced prelicensing inspection and met with Site Supervisor, Jamie Virgil. LPA explained the reason of the inspection and a tour of the center was conducted both inside and outside. The center is located on 43875 Patrick Ave in Coarsegold, CA. This is a change of Ownership from previous Facility -203808119, and is requesting 26 School Age Children ages 6-14 years old. This program will operate year round and have both full-time & part time options from 6:30 AM to 5:30 PM Monday through Friday. This program will operate in classrooms B. Children get brought into the reception area where staff conduct temperature checks and then get escorted to their classroom. Jamie stated that staff and children use masks while indoors.

Jamie stated that the center will provide a morning snack, & afternoon snack. Lunch will be provided by the parents and stated that the center has a plan in place in case a parent forgets to bring their child lunch. There is a kitchen on site in case food needs to be warmed up or stored. There's a high latch in place so children don't have access to it. The knives and sharp utensils are stored in the counter and some cleaning items are stored underneath the kitchen sink. The rest of the cleaning supplies are stored by the children's bathroom in 2 janitor closets which are also closed by a high latch. There is hot running water in the kitchen. Staff will utilize the staff bathroom that's inside the site supervisors office. Ill children will be isolated in the Site supervisors office and utilize the staff bathroom.

The classroom was observed to be clean and free of toxins. The classroom gets cleaned throughout the day and when children are dismissed. LPA observed that the furniture and equipment appeared to be safe and in good condition. There are enough tables, chairs, and plenty of toys for children to play. LPA also reminded Jamie to not store anything on top of high book cases that could potentially fall on children. There is a working carbon monoxide detector inside the kitchen.

Room measurements were taken and reviewed with Jamie. The total inside preschool area measured to an approximate 920 square feet which will accommodate 26 School Age Children. There is adequate storage space available for children's belongings in the classroom. Jamie understands that children's equipment needs to match the capacity she is licensed for. Toys and equipment appear to be age appropriate. There are a total of 2 toilets, 1 Urinal, and 5 sinks/hand washing fixtures which will accommodate the requested capacity of 26 School Age Children. There is a water fountain inside and outside the classroom that children use to have water readily available to them.

The approximate outside square footage for the preschool area is 10,581 which will accommodate the requested capacity of 26 School Age Children. The play area is gated all around and will be shared through staggered times with the Preschool Children. LPA observed age appropriate toys, a bike path, slide, and swings. Applicant is using Sand to cushion fall zones. LPA advised Jamie to get more sand to soften fall zones underneath the swing. There is shade available where children can rest and get water. LPA reminded Jamie to provide adequate supervision at all times and to not commingle licenses.

A fire clearance of 52 children was approved & granted by the Madera County Fire Department on 12/03/2021.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. Jamie was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA also informed Jamie of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following items must be completed prior to issuing a license by 02/08/2022.

Inside Daycare:


· Anchor tall equipment and play items in all classrooms - cubbies, playhouse, etc.
· Get Toilet Caps for all children's toilets.
· Get 4 stools altogether for sinks and toilets.
· Fix wooden boards on the cabinets/drawers in the classroom.
· Get lock for closet next to kitchen area.
· Fix light panels not working properly
· Submit missing licensing documents.

Outside Play Area:


· Wrap wooden poles with cushion material to prevent any injuries
· Repaint all metal equipment so there isn't any rust exposed.
· Fix chain-link fence since links came off at the bottom of the fence and top. All around.
· Get Rain gutters
· Get more sand to cushion fall zones on swing set.

Pending a final file review and completion of above items, a recommendation will be made to license the above facility for a capacity of 26 School Age Children.

LPA reviewed with Jamie the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
