On 9/26/19 at 10:30 A.M., Licensing Program Analyst (LPA) Van met with director, Lisa Woods for an unannounced annual random inspection. The purpose of the inspection was explained, and was granted entry to the facility by the director. This is a Preschool program. The facility has two buildings, a detached Preschool Building #1 and the building #2 located in the Church building. Present today are 49 children with 8 teachers. Staff to children ratio are met. LPA observed that facility has a sign in / out record with full legal signatures from parent or authorized representative. Days and hours of operations are from Monday - Friday 8:00 AM - 4:00 PM.

Along with director, LPA inspected the day care areas for health and safety hazards. The facility is in good repair, free of any hazards, and has proper temperature and ventilation. Variety of age appropriate toys and learning materials for children are available. Classroom is set up for child care; furniture, equipment, sufficient amount of sleeping mats for children are available. All children's chairs and table have rounded corner. There are no pools, spas or other bodies of water at the facility. All bathrooms, toilets and sinks are clean and well sanitized. Per director, there are no firearms or weapons at the center. Facility has smoke detector, carbon monoxide detector in buidling #1, fire extinguishers and a working telephone at the site. LPA did not observe any carbon monoxide detector in the building #2, director states she will purchase a new one and install it. All cleaning solutions, poisons and other chemicals that are dangerous stored inaccessible to children. Sick children will be separated from the group and will be waiting in the director's office while waiting for parents to pick up.

Facility has water pitchers, cups, and water bottle brought from home readily available for children. Per director, This center provides Incidental Medical Service (IMS), LPA reviewed medications storage, and paperwork with director. Director will be sending an updated version of the IMS Plan to licensing. According to director, the center provides snacks, and lunch is provided by parents. LPA observed the food preparation area is clean, organize, and foods store appropriately.

Every licensed child day care center shall have one or more carbon monoxide detectors in the facility that meet the standards established in Chapter 8 (commencing with Section 13260) of Part 2 of Division 12. The department shall account for the presence of these detectors during inspections.

This requirement is not met as evidenced by; Based on observation LPA did not observe any carbon monoxide detector in the second building. Not having a carbon monoxide detector in the building could pose a potential health and safe risks to children in care.

The outside play area is completely fenced, clean and in good repair. The playground is free of hazardous materials. Play equipment is age appropriate, and safe. Climbing structures are securely anchored, free of any loose parts and has sufficient amounts of barks cushioning floor to prevent any fall injuries. There are no bodies of water, shade is provided for children, and the sand box is inspected daily. Drinking water is accessible by water pitchers and cups.

LPA reviewed the staff’s and children’s records. LPA observed that facility has sign in / out record with full legal signatures from parent or authorized representative. Multiple staff members have record of valid Pediatric FirstAid and CPR card in file. There are menus posted at least one week in advance and are visible to the child's authorized representative. LPA also observed facility has posted the required forms (i.e. License, menus, waivers, Notification of Parent's Rights, Notification of Personal Rights, Car Seat Law, and Emergency Disaster Plan). Site Director states Fire and Earthquake drills are done each month, last logged 9/11/19.

LPA reminded the director that as of September 1, 2016, a person may not be employed or volunteer at a child care facility unless he or she has been immunized against influenza, pertussis, and measles. LPA observed the facility has record of required immunization of all staff.

LPA reviewed AB 1207 with the director. As of January 1, 2018, all staff is required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com. LPA observed the training completion certificates of staff in the files.

See 809D for deficiency that was cited today. The copy of this report is reviewed and provided to the director. Notice of site visit is posted and shall remain posted for next 30 days.
