On 4/14/21 at 12:00 pm., Licensing Program Analyst (LPA), Farhan Bashir-Tariq conducted a scheduled PreLicesing inspection for change of location and capacity increase simultaneously. LPA met with Licensee, Maria Duran. Licensee held a small family child care home license previously. Licensee’s previous facility number is 214005484. This old license will be closed upon issuing a new license. This inspection was conducted via Face Time due to COVID-19 restrictions. Purpose of the inspection was explained. LPA verified the background check clearance of adults working or living in the home. Licensee rents the home, which is a three bedroom and two-bathroom house. Hours of operation will be: M-F, 7:30 am- 5:30 pm. Licensee will not provide any food at this time. Children will bring their own food and snacks. Licensee was advised to have some emergency snack supplies available in the home during operational hours. Daycare areas: Bedroom #3( napping only), Bathroom #2, Living room/Play area, Dinning area and Back yard. OFF limit areas: Bedroom #1, Bathroom #1, Bedroom #2, Garage, Front yard, Deck area, Laundry area and Kitchen (except access to back yard).

LPA and Licensee virtually inspected the entire home for health and safety hazards. All off limit areas were properly barricaded and made inaccessible to the children in care. Living room/ Play area will be used as main child care space. LPA observed that there were plenty of toys, books, arts and other learning material available in play area. Windows located in play area and bedrooms were secured with built in locks. There is a small electric heater in living room, which has been blocked off with book shelves. Dinning area is adjacent to play area from one side and with kitchen from other side. Access to kitchen was blocked off with baby gate. Kitchen will remain off limits and children will not have access to kitchen except just to pass and go to back yard periodically. Toxins, Poisons and Chemicals were stored in kitchen cabinets under sink. All harmful and sharp objects were made inaccessible from children in care. License and other required licensing documents were observed to posted on near front door. Back yard is approved and will be utilized for children in care. Back yard is fully fenced all around. There were slides and other playing structures available in the back yard. Artificial grass was installed around slides and other playing equipment to prevent any falls or injuries. Front yard will not be used for children in care and will remain off limits. All electric outlets in play area, bedroom and bathroom were properly covered with child safety covers. Home was observed to be clean and orderly with enough lighting and ventilation. Cubbies were observed in hallway. Home has multiple smoke detectors and a carbon monoxide detector available in the home. Licensee tested one of each detector for this inspection. A working telephone and a fully charged fire extinguisher of size 2A10BC or bigger is also available in the home. First Aid Kit was fully stocked and stored in kitchen. Licensee states that are no firearms or weapons present in home. Per Licensee, there are no bodies of water/swimming pools in the home. Per Licensee, she doesn’t have any pets in the home.

Licensee was aware that she is required to conduct and log a fire and emergency drill once every six months. License confirmed that she will continue to do so. Licensee has a current and valid First Aid and CPR card expiring in December 2022. Licensee has also completed her mandatory child abuse training on 3/13/21.This training is valid for two years. Licensee was reminded of NO walkers, exersaucers, jumpers, bouncers and any similar items to be used for children in care and shall be made inaccessible. Licensee was reminded about the Provider Information Notices (PINs) on the CCLD website. During the inspection, LPA provided information and assistance about issues related to COVID-19. Licensee was reminded to clean and disinfect childcare areas thoroughly at least once a day. LPA observed that posters were posted for COVID-19 symptoms, social distancing, mask requirement and visitors’ policy by the front door. A thermometer, sanitizers, gloves and tissues supplies were observed to be available in the home.

LPA reminded Licensee that all adults, 18 years and older living in the home, helper, or assistant must have finger print clearance and must be associated to the facility by submitting an LIC 9182 with a copy of CDL or CA. ID prior to having any contact with children in care failure to do so could result in an immediate civil penalty. LPA reminded Licensee that as of September 1, 2016, a person may not be employed or volunteer at a childcare facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662. LPA reminded Licensee, as of January 1, 2018, all staff is required to complete Mandated Child Abuse Reporter Training (AB1207) every two years. The training can be obtained online at www.mandatedreporterca.com. LPA encouraged Licensee to frequently visit our website at www.ccld.ca.gov for licensing regulations and new updates. Licensee can also email at childcareadvocatesprogram@dss.ca.gov and ask to be added to the email list for the updates.

This report was discussed and explained to Applicant. Records to be maintained in the facility were discussed and a copy of Form 311D was emailed to Licensee. Mandatory Posting Requirements: License, Emergency Disaster Plan, and Notification of Parent's Rights Poster. Capacity limits of a Large Family Child Care license has been discussed. A handout explaining capacity and ratio limits for a large family child care home was emailed to Licensee along with this report. Department has received fire clearance from San Rafael Fire Department.

Large Family Child Care Home is approved and will be effective as of today, 4/14/21.
