25 | On January 18th, 2022 at 9:30am, Licensing Program Analyst (LPA) Kassandra Medrano met with Center Director, Ilsa Miller and Assistant Director, Criselda Thomas, for a pre-licensing inspection. On 10/25/2022, the facility was hit by a mud slide, which lead to temporary closure, facility has requested temporary operation at this site until they can return to original site. At this time, facility has been notified by the Muir woods park community association that they must relocate by or before summer. The facility will operate as a combination center with preschool (ages 2 years to entry into first grade), toddlers (ages 18-36 months) and infants (ages 6 weeks preschool children (ages 2 years to entry into first grade). The facility was inspected today, indoor and outdoor for health and safety hazards. Indoor: The preschool classroom measures a total of 300 square feet allowing for a capacity of 9 infants. The facility is equipped with a fire extinguisher, smoke and carbon monoxide detectors. There is a separate bathroom for staff. Isolation of ill children will be in bathroom A until parent arrives for pick up. There will be drinking water readily available for children indoors. Facility currently plans to have children sleep on cot for time being. Director states will room will be separated by either divider or curtain for the time being. Outdoor: The outdoor area is will be within the cement front area of facility.Facility will be installing gates to insure, The fire inspection has been conducted and approved as of November 30, 2021. A copy of the STD 850 was received prior to inspection on 12/8/2021. At this time the fire inspector has given facility two week time frame to install fire system into facility. After fire system is installed, inspector will come out to reinspect. the Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. |
