1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Due to Covid19 this visit is done by tele-visit (Zoom). A tele visit was made to the facility by LPA, Mary Trinh. A review of the personnel report on file indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. This is a combination center with a separate preschool and infant/toddler license. The infant/toddler license operates in rooms "Infant Room" and "Infant Nap Room". The preschool license operates in rooms #1, #2, and #3. Operating hours are 7:30 am - 5:30 pm, Mon-Fri. The facility was toured inside and outside and the floor and yard plan were verified. Activity space for infants is separate from other age groups. Infant and toddlers each have separate classrooms and play yards. The "Infant Room" has 432 square feet space. "Infant Nap Room" has 264 square feet space. The outdoor play area has 1050 square feet space. The items which could pose a danger to children (detergents and cleaning compounds) were inaccessible to children. The Director stated that poisons are not stored on site and none were observed. The facility was free of flies, insects and rodents. The toys, floors, and other equipment and surfaces appeared clean, toxic free, safe for infants and in good condition. There is uncontaminated drinking water available to children both indoors and outdoors. The infant changing tables have at least 3" sides and sanitary vinyl pads that are at least 1" thick. The Infant bathroom and hand washing area appeared to be in safe and sanitary operating condition. There was sufficient napping equipment (4 cribs and cots) available that meet requirements. A current menu was posted. Food prep areas are clean. Food is properly stored and refrigerated as needed. A small refrigerator observed for baby milk and bottles. Garbage cans containing solid waste have tight fitting lids. The playground was free of hazards. The playground equipment and surface areas appeared in safe condition. The Director stated no weapons are stored on site and none were observed. Director stated infant/toddler staffing ratios will be met (1:3 ratio) and that 9 infants will be enrolled. The facility was operating within the licensed capacity. At least one staff member present possessed current CPR and First Aid certifications. Full signatures were observed on sign/in, sign-out sheets.
Children’s records shall contain identification forms with authorized representative information, infant feeding plans and Infant Needs and Services Plans. Staff records were reviewed and contained documentation of education and training, as required.

This facility is not providing Incidental Medical Services – IMS. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. This report, as well as the AAP Guide to Safe Sleep Practices brochure, were reviewed and discussed with the Director. All licensing reports are public information and must be made available upon request for at least three years.
Fire clearance was approved by Ukiah Valley Fire District on August 13, 2020. This Infant license is approved for 12 infants as of August 18, 2020.
