25 | LPA Claudia Henley conducted a case management visit. I was met by Director Oliveira. LPA received an incident report which occurred on 8/13/19 at 9:45 a.m.. Interviewed with the Site Supervisor and one staff who was present on the playground at the of the incident. Staff stated that Child #1 was on the monkey bars on the school age playground structure. Witnesses stated that Child #1 was trying to reach over to the next bar and knew he was going to slip, so he let go. Child #1 fell to the ground and landed on his left arm fracturing the left radius bone. Staff witness was not able to reach the child before he fell. Interview with staff and Director stated that there was a teacher near this monkey bar area. In addition, per Director Oliveira, stated that staff are instructed when a child is on the playground structure, they area required to be near the child on the structure to assist if needed. Review of sign in/sign out and employee time sheets showed that there were three staff and 22 children present on the playground at the time of the incident. Director Oliveria stated that the plan of correction in the future is that children who are in 1st grade are not allowed to use this (monkey bar component) on the structure. Director stated that all staff have been advised of this new policy and that all staff are aware of the children's ages and what grade the child is in at school. Director stated that staff will also continue to monitor the playground structure at all times when they are used by children. LPA Henley spoke with Director regarding the liability on this particular component of the school age playground structure and that licensee may want to consider having this playground component removed from the structure area. LPA inspected the climbing structure/monkey bar area. There appeared to be adequate amount of pea gravel under and around the monkey bar area.
BASED UPON INFORMATION THAT WAS OBTAINED FROM THE FACILITY STAFF, IT IS DETERMINED THAT THERE WERE NO VIOLATION OF CCL REGULATIONS. NO FURTHER ACTION IS REQUIRED AT THIS TIME.
