 On 03/11/2022, Licensing Program Analysts (LPA), Roman Iglesias, conducted an unannounced complaint inspection to gather information, investigate the above allegation, and provide findings. LPA met with Master Teacher, Fabiola Reynoso, who accompanied LPA during the indoor/outdoor tour of the facility. LPA took a census and discussed the allegation.
During the course of the investigation, LPA interviewed staff and made observations. While conducting the indoor tour of the facility, LPA observed a bookshelf that was unastable. During the outdoor tour of the facility, LPA observed a tire that was worned out and in the middle of the tire, there was an expose sharp metal.
Based upon observations and information gathered through interviews, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED.

 Per California Code of Regulations, Title 22, Division 12, Chapter 1, the deficiency is being cited on the attached LIC 9099-D.
An exit interview was conducted with Master Teacher, Fabiola Reynoso. A copy of this report and Appeal Rights were provided and discussed with Mrs. Reynoso.
A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days.

 Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times.

This requirement was not met as evidenced by:

LPA observed a bookshelf in the school age classroom that was wobbly.

On this day, Master Teacher removed the bookshelf from the school age classroom and placed it in an area not accessbile to children.

Additionally, staff admitted that the bookshelf had previously fell on top of a child. According to staff, child was not seriously injured, an incident report was completed, and the child's parent was notified.
