25 | On 1/10/22 Licensing Program Analyst (LPA), Joseph Pacheco arrived at the facility to conduct an unannounced Case Management Inspection. LPA met with Principal, Kristi Boesch to discuss an Unusual Incident that was reported to Community Care Licensing on 10/28/21. On that date, the facility reported that Child #1 was returning to the preschool classroom after attending an assembly and walking in a line with the other preschool students and facility staff. Child #1 fell and hit their face on the asphalt. Child #1 had their hands in their pocket at the time when they fell and was unable to break their fall. Child #1 mouth was bleeding from the fall. Staff immediately treated Child #1’s injury and contacted their parents. Child #1 was taken to the school nurse for examination and further treatment. Parents arrived at the facility within minutes of the incident and took Child #1 to Urgent Care and their dentist for further examination. Child #1 returned to care on 10/29/21. No further treatments have been needed. During today’s inspection LPA interviewed staff and inspected the area where the Unusual Incident took place. LPA did not observe any safety hazards or areas of concern. LPA discussed supervision at the time of the incident and it was determined that appropriate supervision was in place.
This appears to be an isolated incident and staff took appropriate measures to address the child’s injury, following appropriate policies, regulations, and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
Exit interview conducted and report was reviewed with the Superintendent, Randall Heller.
This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
