On 12/13/2024 Licensing Program Analyst (LPA) Yesenia Fierro conducted a Case Management inspection that was initiated by licensee. LPA met with Project Secretary, Judy Xiong. LPA conducted a tour of the facility inside and outside. The purpose for the inspection was to inspect the newly added outdoor playground on the elementary grounds. Weaver Union School District (Farmdale Preschool) has requested via a wavier to share the playground with TK and Kindergarten.

During today’s inspection LPA observed the playground consisted of a multi spring bus, small play net, and a park plant that includes three (3) slides, a climbing rock, a climbing ladder, a climbing boulder, a bridge, and a club house. Playground equipment is in good condition, free of sharp, loose, or pointed parts. LPA received the manufacturer documentation that verifies that the newly added play structure is suitable for the ages of 2-5 years of age. The play structure was professionally installed and is properly anchored into the ground. LPA observed adequate cushioning in the form of rubber pour throughout the entire playground. LPA observed the shade structure had paddle around all metal post.

LPA advised for the staff to conduct frequent safety inspections of the play structure to check for any loose parts, cracks, and hazards. Staff is aware of child safety around the play structure and assumes all responsibility. Staff understands that children must be always supervised while using the play structure. Outdoor activity space surface is maintained in a safe condition and is free of hazards. Project Secretary Judy Xiong has the user’s manual and ensures that the manufacturer’s instructions will be followed.

Playground will be ready to use upon Community Care Licensing obtaining a new outdoor facility sketch and a schedule of times for both the AM and PM class and wavier being granted.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. Project Secretary Judy Xiong was provided a copy of appeal rights. A notice of site visit was given and must remain posted for 30 days.
