13 | Licensing Program Analyst (LPA), Cortney Nelson, met with Office Manager, Brandy Griffin-Bagno, and explained purpose of visit- deliver complaint investigation findings for the above allegations. LPA was admitted into the facility by a staff member upon arrival and was advised that Site Director, Heather Diaz, has left early for the day.
LPA Nelson conducted interviews with the Site Director and staff members, as well as observed children during lunch and outside play time. LPA did not observe children injuring other children while playing. Staff members and Site Director interviewed did not observe alleged injuries to child in care. A child received medical care for acute allergic reaction on 8/25/2023 in the evening, however last attended the facility on 8/24/2023. No symptoms of allergic reaction were observed at the preschool, based on the available documentation provided for the child's allergy to tree nuts. The Site Director states the child only consumed food provided by her family while attending the facility. Based on the available evidence, it is concluded that although the allegations listed above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. The allegations are thus UNSUBSTANTIATED.
As a result of this investigation, no deficiencies were cited. Exit interview was conducted and report was reviewed with Office Manager, Brandy Griffin-Bagno.
A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
