25 | Licensing Program Analyst (LPA) Joe Macias conducted an unannounced Case Management Inspection. The purpose of today’s visit is to ensure the facility is in compliance with Title 22 California Code of Regulations. LPA met with the Administrator John Mims, and explained the nature of today's visit.
During the course of a complaint investigation it was determined that the Licensee on record, Barbara Moore, no longer maintains control of property so the license is forfeited. The current operator, John Mims, has been served with a Notice of Operation in Violation of Law (NOVL) and if a completed application is not submitted or operation does not cease within 15 calendar days of the Notice, on day 16, a civil penalty of $200 per day is assessed until a completed application is submitted or operation ceases.
LPA Macias explained that no person, firm, partnership, association, or corporation shall operate, establish, manage, conduct, or maintain a child day care facility in this state without a current valid license therefore as provided in this act. LPA Macias reviewed the requirements for obtaining a child care license with John Mims.
Type A deficiency cited, exit interview conducted and copy of this report provided to the Administrator John Mims. The LPA reviewed the Notice of Operation in Violation of Law with the Administrator John Mims
LPA discussed the requirements of AB633 with the Director and provided the AB633 fact sheet and a copy of Acknowledgement of Receipt of Licensing Reports (LIC 9224), the Director understands the requirements.
A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE FRONT ENTRANCE TO THE CENTER, AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS. |
