25 | Licensing Program Analyst (LPA) Elizabeth Berumen met with Licensee, Veronica Fernandez for an unannounced Required – 1 year annual inspection. LPA was granted access to the home by the Licensee. The Licensee states that she currently has one child enrolled and is waiting for the program she works with to approve more children. LPA observed the required postings, including the facility license, near the front entrance to the home. Days and hours of operation will be Monday - Friday from 6:00 AM to 6:00 PM. There are six adults that reside in the home: Licensee, Licensee's husband (Salvador Valencia), Dulce Fernandez (sister), Salvador Fernandez Jr (son), Cristo Ortiz (son), Cynthia Ortiz Zamora (daughter-in-law). Licensee has a minor nephew who lives in the home. Licensee updated the Application for family child care home LIC279 during today's inspection. LPA requested copies of TB test for Cynthia Ortiz Zamora and Cristo Ortiz.
Child Care Facility Roster and Fire/Disaster drill logs were reviewed during today's inspection. LPA obtained a copy of current roster. Licensee understands the requirement to keep those logs current whenever she enrolls children. Licensee has day care insurance with DCI. Licensee has current CPR and First Aid certifications expiring on March 9, 2025. Licensee has the required vaccines (MMR, Tdap) and has a singed declination for the influenza vaccine. Licensee is current with her Mandated Reporter Training for Child Care Workers which expires on May 26, 2024. LPA reviewed Licensee's file. File was complete with the required forms and immunization records.
LPA toured the indoor and outdoor areas of the home during today's inspection. Licensee has a working telephone in the home (831) 722-4168. Licensee provided her cell phone number (831) 345-5151. The home is clean, orderly, (including proper heating/cooling/ventilation), and safe for the day care children. There are safe & age appropriate toys, play equipment, and materials for the children in the home. LPA observed barricaded stairs inside the home. Off limit areas inside the home: four bed rooms (one bedroom downstairs) and three bedrooms on main floor of the home. Off limit areas outside the home: left side yard, three storage sheds, and upper level parking area. LPA notes that there is a gate barricading the steps leading to the parking lot area. Backyard is safe for children and fenced.
LPA observed a fully charged 3A40BC fire extinguisher which was serviced on March 2023. The smoke/carbon monoxide detectors were tested and proved to be functioning.
No bodies of water were observed. Licensee states that she does not have any weapons in the home. All detergents, cleaning compounds, medications, and other similar items are inaccessible to children. Any poisons are stored in locked storage shed. |
