On Friday, December 9, 2022 at 9:30 AM, Licensing Program Analysts (LPA) Matthew Gallo and Jeremey McClain met with Licensee, Sarah Van Liew, for the purpose of an unannounced annual inspection. Licensee’s two assistants were also present. All individuals subject to criminal background review have obtained a criminal record clearance. 9 children, including 6 preschool children and 3 infants, were present in the facility. Facility hours of operation are Monday through Friday from 7:30 AM to 5:00 PM.

A health and safety evaluation was conducted in all areas accessible to children. Prior to inspection, off-limit areas included: garage and backyard. During visit, Licensee added entire upstairs as an off-limit area. Licensee acknowledged that children may never enter these off-limit areas. LPA observed that the facility is clean, safe, sanitary, and in good repair. LPA observed a functioning smoke detector, carbon monoxide detector, and a full 2A10BC fire extinguisher. The facility has adequate toys that appear to be safe for children to use. The licensee stated there are weapons in the home, and LPAs observed that they were properly stored according to regulation. The backyard is fenced.

LPA reviewed 9 children’s files. All files appeared to be complete. Required postings and the children’s roster were observed. LPA observed current sleep log. Fire drill log was observed. LPA discussed fire drill requirements with licensee. Licensee’s CPR/First Aid card has an expiration date of 3/2024. In the course of reviewing staff files, missing Mandated Reporter training was observed for Licensee's assistant.

LPAS also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Service (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Based on the inspection, one citation has been issued regarding missing Mandated Reporter training. Appeal rights were provided. Exit interview conducted and report was reviewed with the licensee Sarah Van Liew.
