On 3/19/2024, Licensing Program Analyst (LPA) A. Silva conducted an unannounced POC inspection assisted by Licensee Shirani Perera. Upon arrival, total census was 3 preschool children. The facility was operating within ratios. Facility hours are 9:00AM to 2:15PM, Monday through Friday. An on-site Facility Personnel Report Summary review showed that all facility staff or other individuals who require background checks have received criminal record and child abuse index clearances or exemption.

The LPA conducted the visit to verify whether deficiencies related to buildings and grounds and personal rights had been corrected.

During the previous visit on 3/18/24, the LPA observed the classroom was littered with animal fur. The floor and walls were darkened with dirt; dirt accumulated on walls and at the base of the windows. A dry liquid substance left dripping marks on the walls. There was a pungent ammonia-like smell in the classroom characteristic of cat or dog urine. A black cat and a chihuahua dog were observed roaming the classroom.

During today's visit the LPA observed the floor and walls had been wiped clean; the accumulated dirt was removed and the darkened areas of the walls and windows cleaned. The dry liquid stains were also cleaned. The ammonia-like smell was not pungent anymore, but traces of it lingered in the classroom. The cat was still roaming the classroom and beds for pets were placed in two locations in the classroom.

During the previous visit on 3/18/24, the LPA observed animal feces in the playground near the swings and near the monkey bars. The LPA observed clumps of dirt typical of buried cat feces. Flies were observed flying around the feces and the clumps of sand. Children were observed playing around the feces, including one child who was sitting on the sand near where feces were observed by the monkey bars.

During today’s inspection, the LPA observed the feces had been removed from the playground, including the clumps of sand. No flies were observed.

During today’s visit the LPA observed the deficiencies were corrected. The licensee stated staff will clean the classroom every morning to protect the health and safety of children in care. The licensee stated she will be personally be responsible for clearing any feces and other hazardous/foreign materials before the children enter the outdoor activity area to protect the health, safety and personal rights of children in care.

Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with the director.
