During the investigation LPA conducted a physical plant inspection, interviewed 4 staff members, and obtained facility children’s roster, parents handbook, and Covid-19 screening questionnaires. Staff interviewed stated when children get sick, the office is notified, and will bring the child to the isolation room located in the front building until the parent picks up, however with this situation, we didn’t have enough staff in the classroom to get the child to the isolation room and due to Covid-19, we didn’t want to commingle the staff from other classrooms, however the teacher in the classroom separated child #1 (C1) from the other children in the classroom. Through staff interview, it was determined that C1 was recorded with a temperature of 100.5 degrees Fahrenheit and was not taken to the isolation room, but kept separated in the classroom from the other children in care for 20 minutes until another teacher in the classroom came back from their break.
Based on LPA observation, interviews which were conducted and record reviews of children’s roster, parents handbook, and Covid-19 screening questionnaires, the preponderance of evidence was met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22 101226.2 (a) is being cited on the attached LIC 9099D
Exit interview was conducted with Director, Laura, Finley-Sanbrano. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above. |
