During investigation, LPA Tran interviewed 9 staff, 12 children and 5 parents.
During staff interviews, 9 out of 9 interviewed staff denied that they have or have witnessed any staff at the facility, ever taken pictures of the children with the intention to humiliate the children. Teachers stated that they would only take picture of the child with parents' permission and use the picture for communication purposes with the parents.
During children interviews, 6 out of 12 children qualified for the interviews, and 6 out of 6 interviewed children denied witnessing any teacher taking photos of them or other children to make them feel bad.
During parents interviews, 5 out of 5 interviewed parents have no concerns related to the allegation. They are happy with the services the facility is providing for their child/children.
Based on the interviews conducted with 9 staffs, 6 children and 5 parents, there is insufficient evidence to corroborate the allegation that facility humiliated child in front of peers. Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove that facility humiliated child in front of peers did or did not occur, therefore the allegation that facility humiliated child in front of peers is UNSUBSTANTIATED.
Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeal is to Regional Manager, address is above on the report. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Fazreen Shariff.
