25 | Licensing Program Analyst (LPA) Valencia conducted an on site inspection for the purpose of a Case Management Incident inspection, in response to an Unusual Incident Report submitted to the Licensing office on 8/31/21. LPA and the facility representative toured the facility inside and outside, and the floor and yard plan (LIC 999) were verified. Census was taken in individual classrooms. The overall census observed was 78 preschool children with 15 preschool staff.
An Unusual Incident Report UIR LIC624 form was submitted to the Licensing office on 8/31/21 detailing that a parent, adult #1 (see Confidential Names List LIC811) stated to director that their child, child #1 stated to them at home that another child in care, child #2, interacted with C1 inappropriately during nap time. During today's inspection, LPA Valencia conducted a physical plant inspection of the facility, observed the classroom and teacher to child interactions and interviewed parents, children, and staff related to this reported incident. It was determined from the information available to LPA during the inspection, that the facility violated Title 22 Regulations related to supervision. It was determined from the investigation on today's date that for one day, 8/27 one staff, Staff #2, was supervising 21 napping preschool children. During that time, due to limited staffing, some children were obstructed from view, as this S2 needed to supervise two napping classrooms as one time. This led to C1 and C2 interacting inappropriately, without supervision. LPA has reviewed the incident and resources for staffing with the director. Title 22 Regulation section 101229(a)(1) was printed, provided and reviewed with the director. Director also added that they have done their own internal investigation, directed staff to no longer have one staff supervising more than one classroom, no blind spots of cots. Director met with all staff on 8/30 to address supervison in the classrooms. Proof of this meeting was obtained by LPA and is on file.
In the areas that were evaluated, deficiencies were observed of the California Code of Regulations, Title 22, Division 12 section 101229(a)(1) was cited on the attached LIC809D. An exit interview was completed with facility representative and The report was reviewed and discussed. The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. |
