32 | During today, LPA observed how the roll call is done by Staff #3 from the transition from the playground back inside the facility. LPA observed the S3 have the children line up. S3 then called out each child's name and matched the name to the face while checking the child's name off on a roll sheet provided on a clip board.
The facility had a staff training for the facility staff on 03/02/2023 to review accountability of roll taking procedures to ensure the protocol is followed by all staff. The director was informed that the training, meeting should be a common practice and the staff provided reminders throughout the year to ensure the procedure is followed.
Based on LPAs record review and interview the following violation was observed is being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101229(a)(1), 1 Type A is being cited on the attached LIC 809D.
LPA Gutierrez informed the facility Assistant Director, Alma Erwin that this report dated 03/07/2023 documents 1 Type A citation. A Type A citation shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Gutierrez informed the assistant director to provide a copy of this licensing report dated 03/07/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the Alma Erwin, Assistant Director. Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
