On 09/07/2023 Licensing Program Analyst (LPA) Araceli Bootorabi conducted an onsite visit for the purpose of a 1 year required inspection. The LPA and Marcela Garcia toured the facility inside and outside. The floor and yard sketch was reviewed (LIC 999) and verified. Upon arrival, the LPA provided the director with a copy of an Entrance Checklist (LIC 125).

Census was taken in each classroom as was as followed:

Room 1 – Identified as R2 (CC1) – Teachers: 1 Aide: 0 Students 7

Room 2 – Identified as R1 (CC2) – Teaches: 2 Aide:0 Students: 0

The facility was operating within its licensing capacity. The hours of operation are 7am - 6pm


Documents verified to be posted in a prominent, publicly accessible area at the facility. The LPA verified the following documents to be posted in a prominent, and publicly accessible area at the facility license, menus, LIC 613 A Personal Rights, PUB 269 Child Passenger Restraint system poster, PUB 393 Notification of Parent Rights, and LIC 610 Emergency Disaster Plan. The facility met all posting requirements.

Documents to be reviewed during the inspection: Verification of Disaster and Fire Drills, Daily Activity Schedule, Sign-in/out sheets, and LIC 9148 Earthquake Preparedness.



The last drill conducted was 08/10/2023. The facility personnel was reminded of the importance of completing a drill every 6 months and maintaining a log. The facility met the requirements for this section.
The center is using an electronic sign in and out. The director stated that each parent has their own unique log in. The school year began in 08/24/2023 and the facility has not conducted a fire drill for this school semester. The director stated the next drill is planned for 08/13/2023 and a consultation was provided for record keeping drills.

Classroom Walk-Through: The LPA and director visited 2 of 2 classrooms. During the inspection of the indoor activity space, items that could pose a danger to children such as detergents, cleaning compounds, and medications were observed to be stored out of the reach of children in classrooms. The LPA did not observe poisons, and hazardous items such as detergents, cleaning compounds, and medications. The classrooms had age-appropriate toys for the ages served. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges.

Restrooms in R2: The children's restrooms are clean and sanitary. The water temperature was a bit warmer than usual and a consultation was provided. The bathrooms had flushing water. Toilets used by school-age children provide individual privacy.


The director was reminded that all toilets, and hand washing, areas shall be maintained in safe and sanitary operating conditions. The director stated that the children are supervised and a staff member monitors the restrooms/children go to the restroom on their own.

Food Service: The Center programs providing before- and/or after-school care makes available and offers nutritious snacks to children. Food is provided by the facility. The director stated am snack and pm snack is provided.

Large Climbing Equipment: The large climbing structure has a foam like underneath and is maintained by the elementary school where the center is located.

The playground is not fully fenced and enclosed. A consultation was provided for supervision. Director stated that areas are coned off.

There are no bodies of water present at the facility. The outdoor activity area was clean, safe, sanitary, and in good repair during the inspection. The LPA discussed with the director that equipment shall be maintained in a safe condition, free of sharp, loose, or pointed parts.

The director was reminded that the childcare center shall be clean, safe, sanitary, and in good repair at all times to ensure the safety and well-being of children, employees, and visitors.

The director was also reminded that measures to keep the center free of flies, other insects, and rodents. The staff shall safely dispose of water and any disinfectants/solutions that have been used for cleaning. All children shall be protected against hazards within the center by means of properly storing medication, cleaning solutions, and other hazardous items out of the reach of children. All outdoor and indoor passageways, stairways, inclines, ramps, open porches, and other areas of potential hazard shall be kept free of obstruction. Disinfectants, cleaning solutions, poisons, and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. Storage areas for poisons shall be locked. Firearms and other weapons shall not be allowed on or stored on the premises of a childcare center.

Children’s Record were reviewd, and there was a separate, complete, and current record for each child. A random sample of children's files was reviewed for documentation of the child’s name, address, and telephone number of the child’s authorized representative and of relatives or others that can assume responsibility for the child if the authorized representative cannot be reached, when necessary (LIC 700),


LIC 613A Personal Rights, Admission Agreement, LIC 995 Notification of Parents’ Rights, and LIC 627 Consent for Emergency Medical Treatment.
Personnel Record: Personnel records were reviewed at 2:00 pm. The LPA requested a separate, complete, and current record for staff. The following documents must be identified for staff personnel: Staff Qualifications, Proof of immunization of measles, pertussis, and Influenza, Current Pediatric CPR and First Aid Certification (designated staff), TB Clearance or risk assessment, LIC 503 Health Screening Report, LIC 9108 Statement Acknowledging Requirement to report Child Abuse, Mandated Reporter Training Certificate, and LIC 9052 Employee Rights. The facility met the requirements for this section. The LPA reviewed 3 of 3 staff files during today’s visit. 1 of 3 files were missing LIC 503 with TB clearance. A consultation was provided.
A review of the Facility Personnel Report Summary was conducted during today’s inspection. The Facility Personnel Report Summary indicated all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Director was advised that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, before the initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/person will be assessed if this regulation is violated.

Heat Precautions in Child Care: Health and age play a significant role in how people react to extreme heat. Individuals at most significant risk for heat-related illness include infants and children up to four (4) years of age. By knowing how to prepare for a heat wave, and what to do when one occurs, you can avoid the biggest health dangers, such as heat exhaustion and heat stroke. Before sending children out to play, adequate supervision is required and staff need to know how to protect them against heat-related illness. Providers must meet the requirements of the applicable HSC, Child Day Care Act, Sections 1596.70 et seq., and California Code of Regulations, Title 22, Division 12. Have a safe, healthy, and enjoyable summer!

The director understands they must be present in the facility and must ensure children in care are always supervised. Children are not to be left alone in parked vehicles. When the director is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care for and supervise children while absent.

An exit interview was conducted with director Marcela G . Appeal Rights were explained. The Licensee was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First-level appeals should be sent to the regional manager at the address listed above. The Notice of Site Visit was posted and discussed as required by H&S Code Sec. 1596.817. Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.00. The Notice of Site Visit must be posted on or adjacent to the door.
